Clomiphene citrate should be used carefully in certain situations. Be cautious if the treatment lasts a long time, or if the patient drinks alcohol or takes antidepressants. For those with polycystic ovaries, check for ovarian cysts before each treatment cycle. If the patient has uterine fibroids, take extra care. The medicine can sometimes cause vision problems, which may make driving or using machines unsafe. There is also a small chance of having twins or more, so patients should be informed about this risk.
Decreased response with danazol.
Clomiphene citrate should not be used in the following situations: during pregnancy, in people with liver disease, ovarian cysts, unexplained vaginal bleeding, cancer of the endometrium, or if they are allergic to the drug. It is also not safe for people who are breastfeeding, those with uncontrolled thyroid or adrenal problems, or anyone with a brain tumor such as a pituitary tumor.
Clomifene works by blocking the effects of oestrogen (a hormone) on the brain's hypothalamus and pituitary gland. This makes the pituitary gland release more hormones that trigger the ovaries to release eggs, which helps to start ovulation.
Adult: PO: Ovulatory failure: 50 mg/day for 5 days starting on the 5th day of menstrual cycle or at any time if there is amenorrhoea. May increase to 100 mg/day for the 2nd course as early as 30 days after previous therapy, if needed. If this dose induces ovulation there is no need to increase the dose in the following courses.
If this dose induces ovulation but not pregnancy, stimulation with Clomiphene citrate is to be continued up to 6 stimulative courses. For other indications: The recommended dose for other gynaecological problems is 50 mg (1 tab) for 5 days, starting from the 5th day of menstruation or according to the physician's recommendation, but never more than 6 cycles in total.
This medicine must not be used during pregnancy. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict two effective types of birth control starting one month before and during treatment and for three years after stopping this medicine. Discuss with your doctors if you are not sure which types of birth control are effective for you. You must need to take pregnancy tests regularly during your treatment with this medicine and for at least three years after taking this medicine. If it seems that you do become pregnant stop taking this medicine and call your doctor immediately. If you really become pregnant, miss a menstrual period, or have sex without using two forms of birth control. In some cases, your doctor can prescribe emergency contraception to prevent pregnancy. People on this medicine should not donate blood during the treatment and for three years afterward.
This medicine is also contraindicated breast milk. Breastfeeding is not recommended while using this medication and for three years afterward.
