Precautions and warnings

Before taking this medication, tell your doctor if you have any of the following medical conditions: narrow-angle glaucoma, porphyria (a blood disorder), liver disease, kidney disease, breathing problems, mental problems like depression, suicidal thoughts, or a personal or family history of substance abuse.

This medication may cause dizziness or drowsiness. You may become dizzy or sleepy if you consume alcohol or cannabis. Do not drive, operate machinery, or engage in any other activity that requires attentiveness until you are sure you can do so safely. Alcoholic beverages should be avoided. If you use cannabis, talk to your doctor.

Drug Interactions

Orlistat and sodium oxybate are two products that may interact with this medication.

If this drug is combined with other medications that cause drowsiness or breathing issues, the risk of serious side effects such as slow breathing and dizziness may be increased. If you're taking any other medications, such as opioid pain relievers like codeine or hydrocodone, alcohol, cannabis, sleep or anxiety medications (like alprazolam, lorazepam, zolpidem), muscle relaxants (like carisoprodol, cyclobenzaprine), or antihistamines, tell your doctor or pharmacist (such as cetirizine, diphenhydramine).

Contraindications

Acute pulmonary insufficiency, acute narrow-angle glaucoma, hypersensitivity to the active component in benzodiazepines.

Mode of actions

Clonazepam lowers the spike and wave discharge in absence seizures by reducing neuronal transmission in the motor cortex. Its mechanism is thought to be linked to its capacity to boost GABA action. It helps with focal epilepsy and generalized seizures in the clinic.

Dosage & Administration

Adults :1 mg (elderly 500 micrograms) initially at night for 4 nights, increased according to response over 2-4 weeks to usual maintenance dose of 4-8 mg usually at night (may be given in 3-4 divided doses if necessary).

Dosage may be increased in increments of 0.5 to 1 mg every three days until seizures are adequately controlled.

Children: The initial dose for infants and children (up to 10 years of age or 30 kg of body weight) should be between 0.01 and 0.03 mg/kg/day and should not exceed 0.05 mg/kg/day given in two or three divided doses.

Dosage should be increased by not more than 0.25 to 0.50 mg every third day until a maintenance dose of 0.1 to 0.2 mg/kg of body weight has been reached, unless seizures are controlled or side effects preclude further increase. Whenever possible, the daily dose should be divided into three equal doses. If doses are not equally divided, the larger dose should be given before retiring.


IV Emergency treatment of status epilepticus 1 mg as inj or infusion, repeat if needed.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
