Precautions and warnings

Tell your doctor or pharmacist about your medical history, especially if you have kidney illness, liver disease, or a specific form of muscle disease, before using this drug.

Erythromycin has been linked to a heart rhythm disorder. It seldom causes major fast/irregular heartbeat and other symptoms that necessitate immediate medical attention, such as severe dizziness and fainting.

If you have certain medical conditions or are taking other drugs that can induce heart rhythm, your risk of heart rhythm may be raised. Tell your doctor about all of your medications and if you have any of the following problems before using erythromycin: heart failure, a slow heartbeat, and a family history of certain heart diseases are all examples of heart disorders.

Low potassium or magnesium levels in the blood might also put you at risk for an irregular heartbeat. If you use certain drugs, such as diuretics, or if you have problems like extreme sweating, diarrhea, or vomiting, this risk may rise. Consult your doctor about the safe use of erythromycin.

Live bacterial vaccinations, such as the typhoid vaccine, may be affected by erythromycin. Unless your doctor advises otherwise, do not get any vaccines or vaccinations while taking this drug.

Sodium may be present in some erythromycin formulations. If you're on a salt-restricted diet or have a condition like congestive heart failure that could be worse by an increase in salt intake, talk to your doctor.

Drug Interactions

Digoxin is a product that may interact with this medication. Reduces valproic acid levels in the blood, increasing the risk of seizures. Probenecid and increased plasma concentrations.

A variety of medicines, including amiodarone, dofetilide, cisapride, pimozide, procainamide, propafenone, quinidine, and sotalol, might influence cardiac rhythm.

Other drugs may interfere with the elimination of erythromycin from your body, causing erythromycin to operate differently. Azole antifungals like itraconazole, calcium channel blockers like diltiazem, and anti-seizure drugs like carbamazepine, phenytoin, and saquinavir are just a few examples.

Other drugs' clearance from your body may be slowed by erythromycin, which may impact how they work. Bromocriptine, colchicine benzodiazepines, eletriptan, ergot alkaloids, certain medications to treat erectile dysfunction-ED or pulmonary hypertension, and certain statin drugs such as lovastatin, simvastatin, and vinblastine, among others, are among the drugs that are affected.

Contraindications

Hypersensitivity to the active ingredient for porphyria, hepatic impairment, pregnant woman.

Mode of actions

Erythromycin limits protein synthesis by binding irreversibly to the 50S ribosomal subunit, preventing susceptible organisms from transpeptidation or translocation processes, resulting in restricted cell development.

Dosage & Administration

Adults: Usually 250 mg every 6 hours, or 500 mg every 12 hours. May increase up to 4 g/day, according to severity of infection.

Upper respiratory tract infections of mild to moderate severity: 250 to 500 mg 4 times a day for 10 days.
Lower respiratory infections of mild to moderate severity: 250 to 500 mg 4 times a day for 10 days.
Respiratory tract infections due to Mycoplasma pneumoniae: 500 mg 6 hours for 5 to 10 days, treat severe infections for up to 3 weeks.

Skin and soft tissue infections of mild to moderate severity: 250 to 500 mg 4 times a day for 10 days.
Pelvic inflammatory diseases (PID), acute due to Neisseria gonorrhoeae: 500 mg 4 times a day for 10 to 14 days.
Urogenital infection during pregnancy caused by Chlamydia trachomatis: 500 mg 4 times daily for 7 days or
250 mg 4 times daily for 14 days.

Urethral, endocervical or rectal infections, uncomplicated: 500 mg 4 times daily for 7 days or
250 mg 4 times daily for 14 days.

Non-gonococcal urethritis: 500 mg 4 times daily for at least 7 days.
Neisseria gonorrhoeae: Uncomplicated urethral, endocervical or rectal infections and in penicillinase producing N. gonorrhoeae(PPNG): 500 mg 4 times a day for 7 days.

Early syphillis (primary, secondary or latent syphillis of < 1 year duration): 500 mg 4 times a day for 14 days.
Severe or chronic diarrhoea: 500 mg 4 times a day for 7 days. Rheumatic fever: 250 mg 2 times daily.

Bacterial endocarditis: 1 g 2 hours prior to procedures, then 500 mg 6 hours after initial dose.
Acne: 500 mg twice daily for 3 months reduced to 250 mg twice for 3 months.

Children:
The usual oral dose is 30-50 mg/kg body weight per day in divided doses. For more severe infections, the dose may be doubled.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
