Precautions and warnings

Older adults, and people with glaucoma, epilepsy, heart disease, liver problems, asthma, trouble urinating, enlarged prostate, or a blockage in the stomach or intestines should use caution. This medicine may cause drowsiness. For injections (parenteral use), give the drug slowly and with care, because giving it the wrong way can cause serious tissue damage.

Drug Interactions

This drug can hide the hearing damage caused by aminoglycoside antibiotics. It may also make the sedative effects of substances that depress the central nervous system—such as alcohol, sleeping pills, barbiturates, and opioids—stronger.

Warning: Using this drug with other medications that prolong the QT interval can cause dangerous and possibly fatal heart rhythm problems.

Contraindications

Do not use in people who are hypersensitive (allergic), in a coma, or have porphyria, heart disease, or low potassium levels. Avoid giving by intra-arterial or subcutaneous injection. Not recommended for newborns, young children, pregnant or breastfeeding women.

Mode of actions

Promethazine is a type of medicine called a phenothiazine. It works by blocking certain dopamine receptors in the brain and also strongly blocks some adrenaline (alpha-adrenergic) receptors. In addition, it attaches to and blocks H1 histamine receptors.

Dosage & Administration

Oral
Allergic conditions
Adult: As hydrochloride: 25 mg at night inreased to 25 mg bid, if needed; alternatively, 10-20 mg bid or tid.
Child: As hydrochloride: 5-10 yr: 10-25 mg daily; 2-5 yr: 5-15 mg daily. Doses are given in 1 or 2 divided doses.
Renal impairment: Dose reduction may be required.

Prophylaxis of motion sickness
Adult: As hydrochloride: 20 or 25 mg the night before travelling followed by a similar dose in the morning if needed.

Child: As hydrochoride: 5-10 yr: 10 mg; 2-5 yr: 5 mg; doses are given at night before travelling, repeated the following morning if needed. As teoclate: 5-10 yr: 12.5 mg daily starting either on the night before long journeys or 1-2 hr before short journeys.

Renal impairment: Dose reduction may be required.

Short-term management of insomnia
Adult: As hydrochloride: 20-50 mg at night.
Child: As hydrochloride: 5-10 yr: 20-25 mg; 2-5 yr; 15-20 mg. Doses are taken at night. May also be given as suppository.

Renal impairment: Dose reduction may be required.

Parenteral
Allergic conditions
Adult: As hydrochloride: 25-50 mg, given via IM or slow IV inj or injected into the tubing of a freely running infusion at a concentration of not >25 mg/ml and infused at a rate of not >25 mg/min. Max dose: 100 mg.

Child: As hydrochloride: 5-10 yr: 6.25-12.5 mg by deep IM inj.
Renal impairment: Dose reduction may be required.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
