Do not take Tamsulosin Hydrochloride if you are allergic to it, have a history of severe liver problems, or have experienced sudden drops in blood pressure when standing up (orthostatic hypotension). Tamsulosin, like other alpha1 blockers, may sometimes lower blood pressure, which can rarely cause fainting (syncope). If you feel dizzy or weak, sit or lie down until you feel better. Avoid activities that could be dangerous if you faint, such as driving, using machines, or doing risky tasks.
Before starting Tamsulosin, your doctor should check to make sure your symptoms are not caused by something other than Benign Prostatic Hyperplasia (BPH). This may include a physical exam (including a digital rectal examination) and a PSA test, both before treatment and regularly during treatment.
If you have severe kidney problems (creatinine clearance less than 10 ml/min), be extra careful with this medication, as there is limited information about its safety in these patients. Also, if you have porphyria or have had allergic reactions to this or similar drugs, use Tamsulosin with caution.
The level of tamsulosin in the blood can increase if taken with strong CYP3A4 inhibitors like ketoconazole. Moderate CYP3A4 inhibitors (such as erythromycin) or CYP2D6 inhibitors (like strong ones such as paroxetine, or moderate ones like terbinafine) can also raise tamsulosin levels. Taking cimetidine can also increase its concentration. Using tamsulosin with other alpha-blockers can have an added effect. If used together with PDE5 inhibitors, it may cause low blood pressure with symptoms. Taking furosemide can lower tamsulosin levels in the blood.

Tamsulosin is a medicine that blocks specific alpha-1 adrenoreceptors. These receptors are mainly found in the prostate, the outer layer of the prostate, the part of the urethra that runs through the prostate, and the neck of the bladder. Normally, the sympathetic nervous system tightens these muscles. When tamsulosin blocks the receptors, it relaxes these muscles, which helps urine flow more easily and reduces symptoms of benign prostatic hyperplasia (BPH).

Adult: As HCl: As modified-release preparation: 1 (one) capsule once daily is recommended as the dose for the treatment of the signs and symptoms of BPH. It should be administered approximately one-half hour following the same meal each day.
For those patients who fail to respond to the 0.4 mg dose after two to four weeks of dosing, the dose of capsules can be increased to 0.8 mg once daily.
If capsules administration is discontinued or interrupted for several days at either the 0.4 mg or 0.8 mg dose, therapy should be started again with the 0.4 mg once daily dose.

Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
