Precautions and warnings

Do not use this medicine in patients who are experiencing serious health problems that could lead to muscle disease (myopathy) or kidney failure caused by muscle breakdown (rhabdomyolysis). Examples include sepsis (severe infection), low blood pressure, major surgery, serious injuries, severe problems with metabolism, hormones, or body salts, or uncontrolled seizures. Also, avoid using it with drugs like cyclosporine, lopinavir/ritonavir, atazanavir/ritonavir, or certain other medicines that lower cholesterol.

People with a history of liver disease or alcoholism should not use this medicine. Stop treatment if there is a large or ongoing increase in liver enzyme levels, a significant rise in creatine phosphokinase, or signs of muscle disease.

Drug Interactions

This medication may raise the levels of warfarin and oral contraceptives in your blood. It can also increase blood levels when taken with itraconazole or HIV protease inhibitors. However, it may lower the levels if taken with erythromycin or antacids. The risk of muscle problems (myopathy) is higher if taken with fenofibrate or niacin.

Serious Warning: Taking this drug with gemfibrozil or ciclosporin can greatly increase the risk of a dangerous muscle condition called rhabdomyolysis.

Contraindications

Do not use in people with severe kidney problems, active liver disease, or unexplained ongoing high levels of liver enzymes. Also, avoid if you are allergic to the medication. Not recommended during pregnancy or breastfeeding.

Mode of actions

Rosuvastatin works by blocking HMG-CoA reductase, an important enzyme needed to make cholesterol in the body. By doing this, it helps the liver put more LDL receptors on its surface, which leads to more LDL (“bad” cholesterol) being removed from the blood. Rosuvastatin also lowers levels of apolipoprotein B and triglycerides, while raising HDL (“good” cholesterol) levels.

Dosage & Administration

Adult: PO: Dose range: 5-40 mg once daily. Initially 5-10 mg once daily increased if necessary at intervals of at least 4 weeks to 20 mg once daily. Daily dose can be increased after further 4 weeks to 40 mg only in severe hypercholesterolaemia with high cardiovascular risk and under specialist supervision.

Elderly initially 5 mg once daily.

HoFH: Starting dose 20 mg.

Child: Heterozygous familial hypercholesterolaemia: >10 yr Initially, 5 mg once daily, may be adjusted at intervals of at least 4 wk. Max: 20 mg once daily.

Use 40 mg dose only for patients not reaching LDL-C goal with 20 mg Rosuvastatin can be taken with or without food, at any time of day.

Pregnancy & Lactation

This medicine must not be used during pregnancy. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict two effective types of birth control starting one month before and during treatment and for three years after stopping this medicine. Discuss with your doctors if you are not sure which types of birth control are effective for you. You must need to take pregnancy tests regularly during your treatment with this medicine and for at least three years after taking this medicine. If it seems that you do become pregnant stop taking this medicine and call your doctor immediately. If you really become pregnant, miss a menstrual period, or have sex without using two forms of birth control. In some cases, your doctor can prescribe emergency contraception to prevent pregnancy. People on this medicine should not donate blood during the treatment and for three years afterward.

This medicine is also contraindicated breast milk. Breastfeeding is not recommended while using this medication and for three years afterward.
