Precautions and warnings

Lactic acidosis: Lactic acidosis and serious liver enlargement with fat buildup (sometimes fatal) have been reported when using nucleoside analogues, either alone or with other HIV medicines.

Worsening hepatitis after stopping treatment: Some patients who stopped taking anti-hepatitis B medications, like Entecavir, have experienced serious flare-ups of hepatitis B.

Drug Interactions

Drugs that lower kidney function or compete with entecavir for removal from the body may cause higher levels of entecavir or the other drug in the blood.

Contraindications

Entecavir should not be used in people who have had an allergic reaction to Entecavir or any of the ingredients in this medicine.

Mode of actions

This medicine specifically blocks the HBV DNA polymerase enzyme. By doing this, it stops the reverse transcriptase activity, which lowers the amount of viral DNA produced.

Dosage & Administration

The recommended dose of Entecavir for chronic hepatitis B virus infection in nucleoside-treatment-naive adults and adolescents 16 years of age is 0.5 mg once daily.

For Lamivudine-refractory or known Lamivudine or Telbivudine resistance mutations, the recommended dose of Entecavir is 1 mg once daily.

For patients with decompensated liver disease (adult) the recommended dose of Entecavir is 1 mg once daily. Entecavir should be administered on an empty stomach (at least 2 hours after a meal or 2 hours before the next meal).

Dose Adjustment in Renal impairment: Dose adjustment is recommended for patients with creatinine clearance = 50 30 to < 50 10 to < 30 < 10 Hemodialysis or CAPD Recommended dose and dosing interval 0.5 mg every 24 hours 0.5 mg every 48 hours 0.5 mg every 72 hours 0.5 mg every 7 days

Pediatric Use: Safety and effectiveness of Entecavir in pediatric patients below the age of 16 years have not been established.

Geriatric Use: Clinical studies of Entecavir did not include sufficient numbers of subjects aged 65 years and over to determine whether they respond differently from younger subjects. But care should be taken in dose selection, and it may be useful to monitor renal function.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
