Precautions and warnings

Make sure you're getting enough water. For at least one month, use an anti-inflammatory or colchicine as a preventative measure. When sensitivity (skin rash, etc.) arises, stop taking it right away. Lactation and pregnancy. Insufficiency of the liver and kidneys.

Drug Interactions

When administered with allopurinol, it may impede the metabolism of mercaptopurine and azathioprine; therefore, the dose of mercaptopurine and azathioprine should be reduced. Warfarin and other coumarin anticoagulants may be affected. Increased oxypurinol (metabolite) excretion in combination with salicylates or uricosuric agents. Thiazide diuretics, some antibacterials, some antineoplastics, ciclosporin, some sulfonylurea antidiabetics, theophylline, and vidarabine have increased toxicity.

Contraindications

Hypersensitivity; acute attack of gout.

Mode of actions

Allopurinol is a xanthine oxidase inhibitor, which converts hypoxanthine to xanthine and ultimately uric acid. All symptoms of hyperuricemia and gout are relieved when uric acid production is lowered. Inhibition of xanthine oxidase causes an accumulation of its substrates hypoxanthine and xanthine, although there is no risk of nephrolithiasis because their renal clearance is more than 10 times that of uric acid.

Dosage & Administration

Adult: PO Gout; Hyperuricaemia Initial: 100 mg/day, may adjust dose according to response. Max: 800 mg/day.

Prevention of hyperuricaemia associated w/ chemotherapy treatment or enzyme disorders 600-800 mg/day, starting 2-3 days before cancer treatment.

Renal impairment: Haemodialysis: Administer dose after session or administer 50% supplemental dose.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
