People with kidney or liver problems, as well as newborns (both premature and full-term), should have their blood levels checked regularly to prevent harmful side effects.
Iron and vitamin B12 may not work as well in patients with anemia. Phenobarbitone and rifampin make chloramphenicol less effective. It also reduces the effectiveness of birth control pills.
Serious Risks: It can increase the effects of blood thinners, diabetes medicines taken by mouth, and phenytoin (a seizure medication). Do not take this medicine with other drugs that weaken bone marrow.
Do not use this drug if you have had allergic or toxic reactions to it in the past, are pregnant or breastfeeding, or have porphyria. It should not be given by injection for minor infections or prevention. Avoid use if you already have bone marrow problems or blood disorders.
Chloramphenicol stops bacteria from making proteins by attaching to the 50s part of their ribosomes, blocking the enzyme (peptidyl transferase) that forms peptide bonds. It can either slow the growth of bacteria (bacteriostatic) or kill them outright (bactericidal), especially against H. influenzae, N. meningitidis, and S. pneumoniae.
Adult: PO Susceptible infections 50 mg/kg/day in 4 divided doses, may increase to 100 mg/kg/day in more severe infections.
Child: Premature and full-term neonates: 25 mg/kg/day in 4 divided doses. Full-term infants >2 wk: 50 mg/kg/day in 4 divided doses. Children: 50 mg/kg/day in 4 divided doses increased to 100 mg/kg/day for meningitis or severe infections.
Elderly:
Hepatic impairment: Dose reduction may be required.
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
