Precautions and warnings

People with a higher risk of bleeding due to injury, surgery, ulcers, kidney or liver problems, or a history of bleeding or blood-clotting disorders should be cautious. This also applies to pregnant women.

Drug Interactions

Taking this medicine with NSAIDs can raise the risk of stomach and intestinal bleeding. Using a high dose of clopidogrel may also increase warfarin levels in the blood, which can cause more bleeding. High doses of clopidogrel can block the P450 (2C9) enzyme, which can affect how your body breaks down drugs like phenytoin, tamoxifen, torasemide, fluvastatin, and some NSAIDs. You should not use clopidogrel at the same time as drugs that block the CYP2C19 enzyme, such as omeprazole, esomeprazole, cimetidine, fluconazole, ketoconazole, voriconazole, etravirine, felbamate, fluoxetine, fluvoxamine, or ticlopidine.

Contraindications

Do not use if you are allergic (hypersensitive), have active abnormal bleeding, or have been given the medication within 7 days following a heart attack or ischemic stroke. Also avoid if you have bleeding or clotting (coagulation) disorders or are breastfeeding.

Mode of actions

Clopidogrel works by stopping adenosine diphosphate (ADP) from attaching to its receptors on platelets. This prevents the activation of the GP IIb/IIIa complex, which in turn blocks fibrinogen from binding and stops platelets from sticking together and forming clots.

Dosage & Administration

Adult: PO Prophylaxis of thromboembolic events 75 mg once daily.

Acute coronary syndrome For ST-elevation myocardial infarction: W/ aspirin: 75 mg once daily. Continue treatment for up to 28 days.

For unstable angina, non-ST-elevation myocardial infarction: Initial: 300 mg loading dose, followed by 75 mg once daily (w/ aspirin 75-325 mg once daily).

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
