Precautions and warnings
साइक्लोस्पोरिन कैंसर, विशेषकर त्वचा कैंसर और लिंफोमा के विकास के खतरे को बढ़ा सकता है। इस वजह से, रोगियों को सूरज की रोशनी या पराबैंगनी (यूवी) प्रकाश के बहुत अधिक संपर्क से बचना चाहिए। साइक्लोस्पोरिन लेने से बैक्टीरिया, फंगल, परजीवी या वायरल संक्रमण होना भी आसान हो जाता है, इसलिए दीर्घकालिक उपयोग के दौरान विशेष देखभाल और निवारक रणनीतियों की आवश्यकता होती है।
उपचार के पहले कुछ हफ्तों के दौरान, साइक्लोस्पोरिन रक्त में क्रिएटिनिन और यूरिया के स्तर में अस्थायी वृद्धि का कारण बन सकता है, लेकिन अगर खुराक कम कर दी जाए तो इनमें आमतौर पर सुधार होता है। वृद्ध रोगियों को विशेष रूप से अपने गुर्दे के कार्य की बारीकी से निगरानी करने की आवश्यकता होती है। रक्तचाप की भी नियमित जांच होनी चाहिए; यदि उच्च रक्तचाप विकसित होता है, तो इसका उचित उपचार किया जाना चाहिए।
साइक्लोस्पोरिन रक्त में पोटेशियम के स्तर को बढ़ा सकता है, खासकर उन लोगों में जिन्हें पहले से ही किडनी की समस्या है। यदि साइक्लोस्पोरिन को पोटेशियम बढ़ाने वाली अन्य दवाओं के साथ लिया जाता है तो अतिरिक्त सावधानी आवश्यक है। यह मैग्नीशियम के स्तर को भी कम कर सकता है, इसलिए मैग्नीशियम की खुराक की आवश्यकता हो सकती है।
उच्च यूरिक एसिड स्तर वाले रोगियों को साइक्लोस्पोरिन देते समय सावधान रहें। उपचार के दौरान टीके उतने अच्छे से काम नहीं कर सकते हैं और जीवित टीकों का उपयोग नहीं किया जाना चाहिए।
साइक्लोस्पोरिन उन गैर-प्रत्यारोपण रोगियों को नहीं दिया जाना चाहिए जिन्हें किडनी की समस्या, अनियंत्रित उच्च रक्तचाप, चल रहे संक्रमण या कैंसर है। रक्तचाप, गुर्दे और यकृत के कार्य, और रक्त इलेक्ट्रोलाइट्स (जैसे पोटेशियम और मैग्नीशियम) की नियमित निगरानी की आवश्यकता होती है, और डॉक्टरों को यकृत या गुर्दे की समस्याओं, उच्च यूरिक एसिड, एलर्जी या एनाफिलेक्सिस के इतिहास वाले या गर्भवती लोगों में सतर्क रहना चाहिए।

Drug Interactions
कुछ दवाएं आपके शरीर में सिक्लोस्पोरिन के स्तर को बढ़ा सकती हैं, जिनमें डिल्टियाज़ेम, डॉक्सीसाइक्लिन, एरिथ्रोमाइसिन, केटोकोनाज़ोल, मिथाइलप्रेडनिसोलोन की उच्च खुराक, निकार्डिपिन, वेरापामिल और मौखिक गर्भ निरोधक शामिल हैं। दूसरी ओर, कार्बामाज़ेपाइन, आइसोनियाज़िड, फ़ेनोबार्बिटोन, फ़िनाइटोइन और रिफैम्पिसिन जैसी दवाएं सिक्लोस्पोरिन के स्तर को कम कर सकती हैं। सिक्लोस्पोरिन के साथ उच्च खुराक मेथिलप्रेडनिसोलोन का उपयोग करने से दौरे का खतरा बढ़ सकता है।
चेतावनी: अमीनोग्लाइकोसाइड्स, एम्फोटेरिसिन बी, सिप्रोफ्लोक्सासिन, कोल्सीसिन, मेलफ़लान, सह-ट्रिमोक्साज़ोल या एनएसएआईडी सहित विशिष्ट दवाओं के साथ सिक्लोस्पोरिन लेने से गुर्दे की क्षति का खतरा गंभीर रूप से बढ़ सकता है।

Mode of actions

Dosage & Administration
मौखिक
अंग प्रत्यारोपण में इम्यूनोसप्रेशन
वयस्क: प्रारंभ में, 10-15 मिलीग्राम/किग्रा/दिन, प्रक्रिया से 4-12 घंटे पहले शुरू और 1-2 सप्ताह तक जारी; सामान्य रखरखाव: 2-6 मिलीग्राम/किग्रा/दिन। अन्य इम्यूनोसप्रेसेन्ट के साथ मिलाने पर कम खुराक का उपयोग किया जा सकता है।
गंभीर एटोपिक जिल्द की सूजन
वयस्क: प्रारंभ में, 2.5 मिलीग्राम/किग्रा/दिन, 2 विभाजित खुराकों में। एक बार छूट प्राप्त हो जाने पर सबसे कम प्रभावी खुराक कम करें। यदि 6 सप्ताह के भीतर अधिकतम खुराक में पर्याप्त सुधार नहीं होता है तो उपचार बंद कर दें। अधिकतम: 5 मिलीग्राम/किग्रा/दिन।
सोरायसिस
वयस्क: प्रारंभ में, 2.5 मिलीग्राम/किग्रा/दिन, 2 विभाजित खुराकों में। एक बार छूट प्राप्त हो जाने पर सबसे कम प्रभावी खुराक कम करें। यदि 6 सप्ताह के भीतर अधिकतम खुराक में पर्याप्त सुधार नहीं होता है तो उपचार बंद कर दें। अधिकतम: 5 मिलीग्राम/किग्रा/दिन।
रूमेटोइड गठिया
वयस्क: 2.5 मिलीग्राम/किग्रा/दिन, 2 विभाजित खुराकों में। उपचार 6-8 सप्ताह तक जारी रहना चाहिए। यदि प्रतिक्रिया अपर्याप्त है, तो धीरे-धीरे खुराक बढ़ा सकते हैं। अधिकतम: 4 मिलीग्राम/किग्रा/दिन।
नेफ्रोटिक सिंड्रोम
वयस्क: प्रतिदिन 5 मिलीग्राम/किग्रा, 2 विभाजित खुराकों में दिया जाता है।
बच्चा: प्रतिदिन 6 मिलीग्राम/किग्रा, 2 विभाजित खुराकों में दिया जाता है।
अंतःशिरा
अस्थि मज्जा प्रत्यारोपण में ग्राफ्ट अस्वीकृति की रोकथाम
वयस्क: प्रारंभ में, 3-5 मिलीग्राम/किग्रा/दिन, प्रत्यारोपण से एक दिन पहले शुरू होता है और 2 सप्ताह तक जारी रहता है या जब तक 12.5 मिलीग्राम/किग्रा/दिन के रखरखाव पर मौखिक चिकित्सा शुरू नहीं की जा सकती। कम से कम 3-6 महीने तक रखरखाव खुराक जारी रखें।
अंग प्रत्यारोपण में इम्यूनोसप्रेशन
वयस्क: प्रारंभ में: एकल खुराक के रूप में 5-6 मिलीग्राम/किग्रा/दिन, 2-6 घंटे में खुराक डालें। जितनी जल्दी हो सके मौखिक खुराक के रूप में स्विच करें।
Pregnancy & Lactation
यद्यपि पशु प्रजनन अध्ययनों ने भ्रूण पर प्रतिकूल प्रभाव दिखाया है और गर्भवती महिलाओं में कोई पर्याप्त और अच्छी तरह से नियंत्रित अध्ययन नहीं हैं, लेकिन गर्भावस्था के दौरान, संभावित जोखिमों के बावजूद, इस दवा का उपयोग केवल तभी किया जा सकता है जब इसकी आवश्यकता हो।
जिन माताओं के नवजात शिशु हैं, जिन्होंने गर्भावस्था के अंतिम 3 महीनों के दौरान इस दवा का उपयोग किया है, उनमें शायद ही कभी उनींदापन, मांसपेशियों में अकड़न या कंपकंपी, दूध पिलाने या सांस लेने में परेशानी, लगातार रोना जैसे लक्षण पैदा हो सकते हैं। यदि आपको विशेष रूप से उनके पहले महीने के दौरान अपने बच्चे में इनमें से कोई भी लक्षण दिखाई देता है, तो तुरंत डॉक्टर को बताएं।
