Combining this medicine with other aminoglycosides (such as amikacin or streptomycin), cefaloridine, viomycin, polymyxin B, colistin, cisplatin, or vancomycin can increase the risk of damage to the nerves and kidneys. Using it with strong diuretics like ethacrynic acid or furosemide can also make these side effects worse. If given to patients under anesthesia who are also receiving muscle relaxants (such as succinylcholine, tubocurarine, or decamethonium), it may cause longer-lasting breathing problems. The risk of kidney damage is higher if used with ciclosporin or certain other antibiotics (like cephalosporins). Medicines like neostigmine and pyridostigmine may reduce its effects. This medicine can also increase the action of blood thinners like warfarin and phenindione.
This eye drop, like other steroid and antibiotic combination eye medicines, should not be used for most viral infections of the cornea and conjunctiva. This includes conditions such as epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and chickenpox (varicella). It is also not safe for people with mycobacterial or fungal infections in the eye. In addition, people who are allergic or sensitive to any of the ingredients in this eye drop, or to other corticosteroids, should not use it.
Loteprednol is an artificial steroid medicine that is not fluorinated. It works by increasing the amount of special proteins called lipocortins, which help control inflammation by affecting certain chemicals in the body.
Tobramycin is an antibiotic that works by attaching to part of the bacteria called the 30S ribosomal subunit, which stops the bacteria from making proteins they need to grow. It is effective against many types of bacteria that need oxygen to survive, including some that are gram-positive and many that are gram-negative. However, it does not work against Chlamydia, fungi, viruses, or most bacteria that do not need oxygen.
