Precautions and warnings

**Hypoglycemia:**
All sulfonylurea drugs can cause serious low blood sugar. To prevent this, it’s important to choose the right patients, use the correct dose, and give clear instructions. People who are weak, undernourished, or who have problems with their adrenal, pituitary, kidneys, or liver are more likely to have low blood sugar from these drugs.

**Loss of Blood Sugar Control:**
If a patient who is stable on diabetes medicine experiences stress—such as fever, injury, infection, or surgery—their blood sugar may temporarily become harder to control.

**Edema (Swelling):**
The combination of Glimepiride and Rosiglitazone should be used carefully in patients with swelling. Rosiglitazone can cause the body to hold onto fluid, which may make heart failure worse. So, this combination must be used with caution in people who are at risk for heart failure.

**Weight Gain:**
Rosiglitazone, alone or with other blood sugar-lowering drugs, can cause weight gain in a dose-dependent way (the higher the dose, the more likely the weight gain).

**Liver Effects:**
Liver tests should be done before starting treatment with Glimepiride and Rosiglitazone together, and then checked regularly afterward, as advised by your healthcare provider.

Drug Interactions

**Glimepiride:**
Certain medications can increase the blood sugar-lowering effects of glimepiride, making it more potent. These include salicylates (like aspirin), sulfonamides, chloramphenicol, clarithromycin, blood thinners such as coumarins, probenecid, drugs that inhibit CYP2C9, fibric acid derivatives, disopyramide, fluoxetine, quinolones, ACE inhibitors, MAO inhibitors, and some beta-blockers.

On the other hand, medicines such as thiazide diuretics and other water pills, corticosteroids, phenothiazines, thyroid medicines, estrogen, phenytoin, nicotinic acid, sympathomimetics, and CYP2C9 inducers can make glimepiride less effective at lowering blood sugar.

Taking glimepiride with colesevelam can lower the amount of glimepiride in your bloodstream.

**Rosiglitazone:**
Taking rosiglitazone with gemfibrozil can increase the amount of rosiglitazone in your blood. Taking it with rifampicin can decrease its levels.

Contraindications

This treatment should not be used in patients who already have Class III or IV heart failure.

Mode of actions

**Glimepiride:**
Glimepiride works mainly by helping the pancreas release more insulin. It may also reduce how much glucose the liver makes and help the body’s cells use insulin better.

**Rosiglitazone:**
Rosiglitazone helps lower blood sugar by making the body’s cells respond better to insulin, without making the pancreas produce more insulin. It works by turning on a specific receptor in cells (called PPAR-gamma), which affects how the body handles sugars and fats.

Dosage & Administration

The usual starting dose of 1mg/4 mg or 2 mg/4 mg once daily.
The maximum recommended daily dose of is 4 mg of Glimepiride and 8 mg of Rosiglitazone.
The dosage of anti-diabetic therapy should be individualized on the basis of effectiveness and tolerability.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
