Precautions and warnings

Before using this drug, inform your doctor or pharmacist about any previous medical conditions you've had, especially if you've had seizures, a head injury, or a tumor.

Headaches, numbness or tingling of the skin, and seizures are all possible side effects of this medication. Do not drive, operate machinery, or engage in any activity that necessitates alertness or clear vision until you are certain you can do so safely.

This drug contains sodium in some versions. If you're on a salt-restricted diet or have a condition that could get worse if you eat extra salt, talk to your doctor.

Meropenem may impair the effectiveness of live bacterial vaccinations. Before getting any immunizations or vaccinations, tell your doctor that you're taking meropenem.

Side-effects

  • nausea
  • vomiting
  • abdominal pain; headache; constipation; rash
  • pruritus
  • uticaria; apnoea; phlebitis
  • thrombophlebitis
  • swelling
  • pain at inj site; disturbances in LFTs (may cause increases in serum transaminases
  • alkaline phosphatase
  • lactic dehydrogenase). Rarely: erythema multiforme; eosinophilia
  • thrombocytopenia
  • leucopenia
  • neutropenia; seizures
  • CNS effects reported in patients with underlying CNS disorders or renal impairment. Potentially Fatal: Anaphylaxis; pseudomembranous colitis; Stevens-Johnsons sydrome.

Drug Interactions

Valproic acid and similar medications are examples of products that may interact with this medication.

Mode of actions

Meropenem is a carbapenem lactam antibiotic that works by binding to numerous penicillin-binding proteins to suppress bacterial cell wall formation in gm+ve and gm-ve bacteria (PBPs).

Dosage & Administration

Adult: IV Susceptible infections 0.5-1 g 8 hrly. Meningitis 2 g 8 hrly. Cystic fibrosis Up to 2 g 8 hrly. Skin infections 500 mg 8 hrly. Diabetic foot; Intra-abdominal infections 1 g 8 hrly.

Children: 3 months to 12 years. 10 to 40 mg/kg intravenously every 8 hours depending on type and severity of infection, susceptibility of the pathogen(s) and the condition of the patient.
Intra-abdominal infections: 20 mg/kg every 8 hours.
Cystic fibrosis (4-18 years): 25-40 mg/kg every 8 hours.
Meningitis: 40 mg/kg IV every 8 hours.

Children over 50 kg weight: use adult dosage.

Renal impairment:
CrCl (ml/min)
26-50 Usual dose 12 hrly.
10-25 Half the usual dose 12 hrly.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
