Drug Interactions

Certain drugs, called CYP450 inducers (like phenobarbital, phenytoin, carbamazepine, rifampicin, rifabutin, nevirapine, efavirenz, tetracyclines, ampicillin, oxacillin, and co-trimoxazole), can lower the concentration of this medication in your body. Ritonavir and nelfinavir, which are usually CYP450 inhibitors, can also act as inducers when taken with steroid hormones. Taking this medicine with NSAIDs or vasodilators may cause your body to retain more fluid. If you are on medication for diabetes, thyroid problems, or blood thinners, your doctor may need to change your dose.

Warning: This drug can dangerously increase the concentration of ciclosporin in your body.

Contraindications

Serious liver problems, unexplained vaginal bleeding, porphyria, pregnancy, a history of unexplained or current blood clots, clotting disorders, or deep vein thrombosis (DVT).

Mode of actions

Norethisterone acts like a typical progestogen and changes the lining of the uterus from its growth phase to its secretory phase. While it might also have some mild effects similar to estrogen, androgens, or anabolic agents, these are usually not very important. Norethisterone can be used to delay periods and manage irregular or heavy uterine bleeding. It also works as a contraceptive by stopping ovulation using negative feedback on hormones released by the pituitary gland.

Dosage & Administration

Adult: PO Metropathia haemorrhagica (dysfunctional uterine bleeding): 1 tablet 3 times daily for 10 days. Menstruation occurs within 2-4 days after discontinuing treatment.

Prophylaxis against recurrence of dysfunctional bleeding: If there are no signs of resumption of normal ovarian function recurrence must be anticipated. Cyclical bleeding can be established with 1 tablet twice daily from the 19th to the 26th day of the cycle.

Premenstrual syndrome (including premenstrual mastalgia): 2-3 tablets daily from the 19th to the 26th day of the cycle. Treatment should be repeated for several cycles.

Postponement of menstruation: 1 tablet three times daily, starting 3 days before the expected onset of menstruation. A normal period should occur 2-3 days after the patient has stopped taking tablets.

Endometriosis (pseudo-pregnancy therapy): Long-term treatment is commenced on the 5th day of the cycle with 2 tablets daily for the first few weeks. In the event of spotting, the dosage is increased to 4, and, if necessary, 5 tablets daily. After bleeding has ceased, the initial dose is usually sufficient.

Menorrhagia (hypermenorrhoea): 1 tablet 2-3 times a day from the 19th to the 26th day of the cycle (counting the first day of menstruation as day 1).

Pregnancy & Lactation

This medicine must not be used during pregnancy. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict two effective types of birth control starting one month before and during treatment and for three years after stopping this medicine. Discuss with your doctors if you are not sure which types of birth control are effective for you. You must need to take pregnancy tests regularly during your treatment with this medicine and for at least three years after taking this medicine. If it seems that you do become pregnant stop taking this medicine and call your doctor immediately. If you really become pregnant, miss a menstrual period, or have sex without using two forms of birth control. In some cases, your doctor can prescribe emergency contraception to prevent pregnancy. People on this medicine should not donate blood during the treatment and for three years afterward.

This medicine is also contraindicated breast milk. Breastfeeding is not recommended while using this medication and for three years afterward.
