This medication should be used with caution in people with conditions such as hyperthyroidism, heart failure, heart disease, irregular heartbeats, recent heart attack, or high blood pressure. Care is also needed for pregnant or breastfeeding women, the elderly, and people with fever, diarrhea, acid reflux, or ulcerative colitis. Use caution if the liver or kidneys are not working well. There is a higher risk of fever or heat stroke if the patient is in hot environments. Extra care should be taken when giving this medication to people with Down syndrome.
This medicine has stronger anticholinergic side effects when taken with MAOIs, certain antidepressants (TCAs), phenothiazines, antihistamines, and disopyramide. It can slow down stomach emptying, which may delay how other drugs are absorbed. It could reduce the effects of domperidone, cisapride, and metoclopramide on the digestive system. It may also decrease how well ketoconazole and levodopa are absorbed, but increase the absorption of nitrofurantoin. This drug might raise the levels of digoxin in the blood. It could lower the effectiveness of haloperidol and nitrate tablets taken under the tongue. Lastly, it may weaken the effects of parasympathomimetic drugs.
Glaucoma, blockages in the stomach or urinary tract, severe ulcerative colitis or toxic megacolon, weak or inactive intestines, hiatus hernia with acid reflux, unstable heart or blood pressure during heavy bleeding, enlarged prostate, myasthenia gravis, paralysis of the intestines (paralytic ileus), narrowing of the stomach outlet (pyloric stenosis), and angle-closure glaucoma.

Oral
Adjunct in peptic ulcer
Adult: 15 mg tid and 30 mg at bedtime. Doses up to 120 mg daily may be needed.
Child: For relief of GI spasm: 1 mth - 12 yr: 300 mcg/kg (max 15 mg), given 3-4 times daily.
Elderly: 7.5 mg tid. May increase to a max of 30 mg tid.
Gastrointestinal tract spasm
Adult: 15 mg tid and 30 mg at bedtime. Doses up to 120 mg daily may be needed.
Child: For relief of GI spasm: 1 mth - 12 yr: 300 mcg/kg (max 15 mg), given 3-4 times daily.
Elderly: 7.5 mg tid. May increase to a max of 30 mg tid.
Urinary incontinence
Adult: 15-30 mg bid/tid.
Adult enuresis
Adult: 15-30 mg bid/tid.
Hyperhidrosis
Adult: 15-30 mg bid/tid.
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
