Some medicines can change the way repaglinide works in your body. Drugs like rifampicin, barbiturates, and carbamazepine can make your body break down repaglinide faster. On the other hand, medicines such as NSAIDs, salicylates, sulphonamides, phenylbutazone, blood thinners, and hydantoins can make repaglinide work more strongly. Drugs like ketoconazole, fluconazole, itraconazole, and erythromycin can raise the amount of repaglinide in your blood. Medicines that make your blood sugar go up can reduce the effect of repaglinide. If you use gemfibrozil with repaglinide, it can cause your blood sugar to drop too much and for too long.
**Warning:** Using repaglinide with isophane insulin can greatly increase the risk of having a heart attack.
Do not use if you have diabetic ketoacidosis, severe liver problems, type 1 diabetes, or an allergy to this medication. Do not use while breastfeeding.
Repaglinide helps the pancreas release more insulin by blocking potassium (K) from leaving the beta cells. This action closes special potassium channels controlled by ATP, making the cell’s electrical charge change (depolarize). As a result, calcium channels open, letting calcium enter the cell. The increase in calcium then triggers the beta cells to release insulin.
Adult: PO Initial: 0.5 mg before main meals. May adjust dose 1-2 wkly, up to 4 mg/dose. Max: 16 mg/day.
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
