Precautions and warnings

Female patients should abstain from alcohol for the duration of their treatment and for two months afterward. Don't give blood while you're on therapy or for at least a year after you've finished. Children. For long-term therapy, radiographic treatment is used. The treatment should not last more than six months. Regularly check plasma cholesterol and glucose levels (particularly in diabetics). Keep an eye on your LFTs.

Side-effects

  • alopoecia; CNS disturbances; sweating; taste disturbance
  • gingivitis; benign intracranial hypertension; photosensitivity; skeletal hyperostosis; extraosseous calcification; premature epiphyseal closure in child. Potentially Fatal: Phototoxicity
  • jaundice
  • hepatitis
  • hepatotoxity.

Drug Interactions

When taking keratolytics or a high dose of vitamin A at the same time. Reduces the anticoagulant properties of coumarins like warfarin. Oral contraceptives containing microdised progestin. Hepatotoxicity can be exacerbated by taking methotrexate at the same time. Tetracycline should not be taken at the same time.

Contraindications

Pregnancy (before the start, during and at least 2-3 year after cessation of therapy), lactation. Hepatic and renal impairment. Hyperlipidaemia.

Mode of actions

Acitretin is an active metabolite of etretinate but its mechanism of action is unknown.

Dosage & Administration

Oral
Darier's disease
Adult: Initially, 10 mg daily for 2-4 wk. Max: 50 mg/day.

Congenital icthyosis
Adult: Initially, 25-30 mg daily for 2-4 wk before dose adjustments are done. Usual range: 25-50 mg daily for further 6-8 wk. Max: 75 mg daily for short periods.

Severe psoriasis
Adult: Initially, 25-30 mg daily for 2-4 wk before dose adjustments are done. Usual range: 25-50 mg daily for further 6-8 wk. Max: 75 mg daily for short periods.

Severe lichen planus
Adult: Initially, 25-30 mg daily for 2-4 wk before dose adjustments are done. Usual range: 25-50 mg daily for further 6-8 wk. Max: 75 mg daily for short periods.

Child: 500 mcg/kg/day. Max: 35 mg daily.

Pregnancy & Lactation

This medicine must not be used during pregnancy. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict two effective types of birth control starting one month before and during treatment and for three years after stopping this medicine. Discuss with your doctors if you are not sure which types of birth control are effective for you. You must need to take pregnancy tests regularly during your treatment with this medicine and for at least three years after taking this medicine. If it seems that you do become pregnant stop taking this medicine and call your doctor immediately. If you really become pregnant, miss a menstrual period, or have sex without using two forms of birth control. In some cases, your doctor can prescribe emergency contraception to prevent pregnancy. People on this medicine should not donate blood during the treatment and for three years afterward.

This medicine is also contraindicated breast milk. Breastfeeding is not recommended while using this medication and for three years afterward.
