Liver or kidney problems, or weak bone marrow. Use this medicine with mesna and make sure to drink plenty of fluids (by mouth or IV) to help prevent bladder side effects.
Ifosfamide can be more toxic when taken with allopurinol or cisplatin. It can also make warfarin work more strongly, increasing its blood-thinning effects. Some drugs that speed up the CYP2A6 enzyme, such as amobarbital, phenobarbital, pentobarbital, rifampin, and secobarbital, may lower the amount of ifosfamide in your blood. In contrast, drugs that slow down this enzyme, like isoniazid, methoxsalen, and miconazole, can raise ifosfamide levels.
Similarly, drugs that increase CYP3A4 activity — such as aminoglutethimide, carbamazepine, nafcillin, nevirapine, phenobarbital, phenytoin, and rifamycins — may also lower ifosfamide levels in the blood. On the other hand, drugs that block CYP3A4 — like azole antifungals, clarithromycin, diclofenac, doxycycline, erythromycin, imatinib, and isoniazid — may raise ifosfamide levels.
Hypersensitivity; severe bone-marrow depression. Pregnancy, lactation.
Ifosfamide is changed by liver enzymes into active forms that work as alkylating agents, which interfere with the DNA and protein production in target cells. To help prevent damage to the bladder and urinary tract, it is usually given together with mesna.
