Precautions and warnings

Tell your doctor if you have any of the following medical conditions: kidney disease, liver disease, certain blood disorders, a history of blood disorders caused by trimethoprim or sulfa medications, vitamin deficiency (folate or folic acid), severe allergies, asthma, decreased bone marrow function, a metabolic disorder such as high level of potassium or low level of sodium in the blood.

This medicine may impair the effectiveness of live bacterial vaccinations such as the typhoid vaccine. Before getting any vaccines or vaccinations, inform your doctor that you are taking this drug.

Side-effects

Drug Interactions

"Blood thinners" (such as warfarin), dofetilide, methenamine, and methotrexate are some of the products that may interact with this medicine.

Co-admin with pyrimethamine can lead to megaloblastic anemia, which can be fatal. Cyclosporin causes kidney damage to be worsened.

Contraindications

Patient should avoid if it has hypersensitivity to the active ingredient, severe renal or hepatic insufficiency.

Mode of actions

The synergistic activities of its components (sulfamethoxazole and trimethoprim) are multiplied by ten in co-trimoxazole. Sulfamethoxazole prevents the creation of dihydrofolic acid from PABA, preventing bacterial folic acid synthesis and development. Trimethoprim blocks the folic acid pathway enzymes, preventing the dihydrolic acid from converting to tetrahydrofolate. E coli, Klebsiella spp., Enterobacter spp., M morganii, P mirabilis, P vulgaris, H influenzae, Strep pneumoniae, Pneumocystis (carinii) jiroveci, and Cyclospora spp. are all susceptible to co-trimoxazole.

Dosage & Administration

Adult: PO Tablet : 2 tablet twice daily. In severe cases, 3 times/day. DS Tablet : 1 tablet twice daily.
Suspension : 1-2 teaspoonful twice daily.

GI infections; Resp and urinary tract infections; Susceptible infections 960 mg twice daily, up to 2.88 g/day in 2 divided doses for severe cases.

Pneumocystis (carinii) jiroveci pneumonia Up to 120 mg/kg/day in 2-4 divided doses for 14-21 days. Infection prophylaxis in AIDS patients 960 mg/day.

Child: 6 wk-5 mth: 120 mg bid; 6 mth-5 yr: 240 mg bid; 6-12 yr: 480 mg bid.

Renal impairment: Dosage reduction for adults and children >12 yr:
CrCl (ml/min)
15-30 Half the standard dose.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
