Precautions and warnings

When using Tacrolimus ointment to treat patients with atopic dermatitis who are likely to get skin infections, caution is needed. The safety of Tacrolimus ointment has not been proven for people with widespread redness of the skin (generalized erythroderma).

Drug Interactions

Tacrolimus can be more harmful to the kidneys if taken with other drugs like ciclosporin, aminoglycosides, amphotericin B, cisplatin, NSAIDs, vancomycin, co-trimoxazole, aciclovir, or ganciclovir. There is also a higher risk of high potassium levels when used with potassium-sparing diuretics. Some medicines—such as azole antifungals, calcium channel blockers, cimetidine, danazol, HIV-protease inhibitors, macrolide antibiotics, and metoclopramide—increase tacrolimus levels in the blood, which can increase side effects. On the other hand, drugs like antacids, rifampin, rifabutin, caspofungin, phenytoin, phenobarbital, and carbamazepine can lower tacrolimus levels, making it less effective. Taking sirolimus and tacrolimus together can lower the levels of both drugs.

Contraindications

Tacrolimus ointment should not be used by people who are allergic to Tacrolimus or any other ingredient in the ointment.

Mode of actions

Tacrolimus works by stopping T-lymphocytes (a type of immune cell) from becoming active, although its exact method isn't fully understood. It attaches to certain proteins inside cells called immunophilins, like FK binding protein-12 (FKBP-12). This connection creates complexes that block the creation of signaling proteins (cytokines) through a pathway called calcineurin. By blocking calcineurin, tacrolimus prevents the T-cells from activating early, which leads to reduced immune activity (immunosuppression).

Dosage & Administration

Apply a thin layer of Tacrolim ointment onto the affected skin areas twice daily and rub in gently and completely. Treatment should be continued for one week after clearing of signs and symptoms of atopic dermatitis. The safety of Tacrolim ointment under occlusion which may promote systemic exposure has not been evaluated. Tacrolim ointment should not be used with occlusive dressings.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
