Liver or kidney problems. Use with caution in children, older adults, pregnant women, and those who are breastfeeding. Check liver function tests regularly. If liver enzymes stay more than three times above the normal range, stop the medication. Do not stop the treatment suddenly.
Tizanidine can lower blood pressure even more when used with antihypertensive medications. Its effects on the brain and nervous system are stronger if taken with alcohol or other medications that depress the central nervous system. The risk of slow heart rate (bradycardia) increases if taken with beta-blockers or digoxin. Tizanidine levels in the blood go up if it is used with drugs that block CYP1A2, such as zileuton, ciprofloxacin (and other fluoroquinolones), certain antiarrhythmics, cimetidine, famotidine, birth control pills, acyclovir, and ticlopidine. Tizanidine levels can decrease if used with rifampicin. Be careful using tizanidine with drugs that may prolong the QT interval, as this can affect heart rhythm.
Tizanidine is a muscle relaxant that works mainly in the brain and spinal cord. It is similar in structure to clonidine and belongs to a group of medicines called alpha2-adrenergic agonists. Tizanidine helps reduce muscle spasticity by blocking certain nerve signals before they reach the muscles. It does not act directly on the muscles, the connections between nerves and muscles, or simple spinal reflexes.
Adult: PO Spasticity >18 yr: Initial: 2 mg once daily, increase slowly if needed, up to 24 mg/day in 3-4 divided doses. Max: 24 mg/day; 8 mg/dose.
Painful muscle spasm associated w/ musculoskeletal conditions >18 yr: 2-4 mg 3 times/day.
Elderly: Not recommended.

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
