People with liver disease risk factors (especially women, those who are overweight, pregnant, or exposed for a long time) should be monitored carefully. Stop treatment if there are any signs or lab results suggesting lactic acidosis or liver damage. Pause the medication if muscle problems (myopathy) are suspected, and stop it completely if myopathy is confirmed. Keep checking liver function tests (LFTs) for several months after stopping treatment, as there is a risk of sudden, severe worsening of hepatitis B. Take extra care if the patient has kidney problems. The safety and effectiveness of this treatment in these patients are not yet known.
When used together with peginterferon alfa-2a, there is a higher chance of developing peripheral neuropathy.
Telbivudine is a man-made medicine similar to the natural substance thymidine. After entering the body, it is changed inside cells into its active form, called telbivudine triphosphate. This active form works by blocking an enzyme (DNA polymerase) that the hepatitis B virus needs to make more of its DNA. By stopping this enzyme, telbivudine helps reduce the virus’s ability to multiply. It is used to treat long-term (chronic) hepatitis B infections.
