Femipil is indicated for

Precautions and warnings

Do not use in people with sex hormone-dependent cancers, a history of ectopic pregnancy, or problems absorbing nutrients (malabsorption syndromes). Avoid if there are functional ovarian cysts, active liver disease, repeated episodes of jaundice (especially during pregnancy), heart or kidney problems, diabetes, asthma, epilepsy, migraines, or depression. Use caution if breastfeeding or if conditions that worsen with water retention are present. Be careful with high calcium levels, heart or gallbladder diseases, unusual blood fat levels (lipid effects), inherited problems with fat metabolism, or risks of blood clots, breast cancer, existing fibroids, or benign liver tumors. Also, avoid if there is a family history of artery disease, high blood pressure (140/90 mmHg or above), age over 35, or BMI between 30–39 kg/m². Migraines without aura, controlled by 5HT1 drugs, also require caution. Vomiting, diarrhea, missed pills, or other drug interactions may reduce effectiveness, so extra birth control might be needed. Take the medication at the same time every day.

  • absence of withdrawal bleeding; rarely photosensitivity; increased risk in breast cancer; elevation of plasma bound iodine
  • cortisol
  • thyroid binding
  • erythrocyte sedimentation may be accelerated; increases in plasma copper
  • iron
  • alkaline phosphatase; may affect serum triglyceride
  • lipoprotein levels; retinal vascular thrombosis. Potentially Fatal: Hepatic tumours; increased risk of thromboembolism.

Drug Interactions

Some medications can make contraceptives less effective. These include certain antibiotics, anticonvulsants, and drugs that speed up how quickly your body clears contraceptive hormones. Examples are bosentan, rifampicin, rifabutin, barbiturates, primidone, phenytoin, carbamazepine, oxcarbazepine, topiramate, griseofulvin, and aprepitant.

Using troleandomycin with contraceptives may cause severe itching and jaundice, so do not use them together.

The drug ursodeoxycholic acid works less well when taken with contraceptives because contraceptives increase the removal of cholesterol in bile.

Medications like danazol or gestrinone can interact with hormonal contraceptives, making both less effective, so avoid using them at the same time.

Anti-HIV protease inhibitors can also lower how well contraceptives work. Ethinyl estradiol (a type of contraceptive) can raise levels of tacrolimus in the blood, and oral contraceptives can raise the levels of theophylline, selegiline, and tizanidine.

Contraindications

**Avoid use in the following situations:**

- Currently pregnant or have unexplained vaginal bleeding
- Severe artery disease, or a family history of high cholesterol problems
- Liver tumors or liver problems
- History of breast cancer (except for certain cases with metastatic disease)
- Had or have blood clots or clotting disorders
- Estrogen-dependent tumors
- Smoking 40 or more cigarettes daily
- Older than 50 years
- Diabetes with complications
- BMI over 39 kg/m² (severely obese)
- Migraines with aura that last more than 72 hours even with treatment, or migraines treated with ergot drugs
- Very high blood pressure (over 160/100 mmHg)
- Past transient ischaemic attacks (mini-strokes) without headaches
- Systemic lupus erythematosus (SLE)
- Gallstones
- History of haemolytic uraemic syndrome
- Severe itching during pregnancy (pruritus)
- Cholestatic jaundice (jaundice caused by blocked bile flow)
- Severe movement disorders (like chorea) or worsening ear bone problems (otosclerosis pemphigoid)
- Breastfeeding within the first 6 months after childbirth
- After removal of a hydatidiform mole (a type of pregnancy-related tumor)
- Porphyria (a rare blood disorder)

Mode of actions

Norgestrel is a type of progestogen made from nortestosterone, and ethinylestradiol is an artificial form of estrogen. When these two are taken together, they prevent ovulation and cause changes in the cervical mucus and the lining of the uterus, making it harder for pregnancy to occur.

Dosage & Administration

Adult: PO 1 tab once daily.

Pregnancy & Lactation

FDA has not yet classified this medicine into a specified pregnancy category. Although there is no enough evidence that taking this medicine during you are pregnant is harmful.

Discuss with your doctor if you are trying to get pregnant while taking this medicine too.

If you do become pregnant while you are taking this medicine, you should continue to take it and contact with your doctors as quickly as possible. They can assist you to decide whether or not to keep taking this medicine. If you abandon taking this medicine and become sick, this would be more pernicious to your child than staying on the medication.


There is no specific information about whether this medicine is passed on to the baby through breast milk. During this medication, if your baby becomes very sleepy, restless, fatigued, exhibit feeding problems, stop breastfeeding and talk to your doctors very quickly.
