Precautions and warnings

Be careful when prescribing oral corticosteroids like Prednisolone to patients with certain health problems, such as tuberculosis, high blood pressure, heart failure, liver or kidney problems, diabetes (or a family history of diabetes), osteoporosis, a history of severe mood or mental health issues (especially if they have had steroid-related mental problems before), epilepsy, stomach ulcers, or past muscle problems caused by steroids.

To reduce side effects, use the lowest dose that works, for the shortest time possible. If possible, take the full daily dose in the morning and, if appropriate, take it every other day. Check your patients often to adjust the dose according to how their condition changes.

Drug Interactions

This medication can reduce the effectiveness of anticholinesterases used for myasthenia gravis. It may also lower the blood sugar-lowering effects of antidiabetic drugs and decrease the amount of salicylates (like aspirin) in your blood. There is a higher risk of low potassium when taken with diuretics (such as thiazides or furosemide), amphotericin B, or some bronchodilators (xanthines, beta-2 agonists).

Using this drug with NSAIDs can increase the chance of stomach bleeding or ulcers. It might make blood thinners (like warfarin) work more strongly. Some medicines (CYP3A4 inducers like carbamazepine, phenobarbital, or rifampin) can reduce this drug’s effect, while others (CYP3A4 inhibitors like azole antifungals, clarithromycin, or erythromycin) can increase its levels and side effects.

It may make vaccines less effective or raise infection risk from live vaccines. Antacids and bile acid sequestrants can reduce how much of this medicine your body absorbs, so take them at least 2 hours apart. There is a greater risk of muscle problems with neuromuscular-blocking drugs and tendon issues with fluoroquinolone antibiotics.

Contraindications

People who have widespread fungal infections or who are allergic to any of the ingredients should not use this.

Mode of actions

Methylprednisolone is a man-made corticosteroid drug that mostly acts like a glucocorticoid, with very little mineralocorticoid effect. It reduces inflammation by stopping certain white blood cells (polymorphonuclear leukocytes) from moving to the site of inflammation and by reversing the increased leakiness of small blood vessels (capillary permeability).

Dosage & Administration

The initial dosage of Methylprednisolone may vary from 5 mg to 60 mg of methylprednisolone per day depending on the specific disease entity being treated.

In situations of less severity lower doses will generally suffice while in selected patients higher initial doses may be required. The initial dosage should be maintained or adjusted until a satisfactory response is noted. If after a reasonable period of time there is a lack of satisfactory clinical response, Methylprednisolone should be discontinued and the patient transferred to other appropriate therapy.

Use in children
Corticosteroids cause growth retardation in infancy, childhood and adolescence, which may be irreversible. Treatment should be administered where possible as a single dose on alternate days.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
