Precautions and warnings
Medicines that affect the renin-angiotensin system can harm or even kill a fetus if taken during the 2nd and 3rd trimesters of pregnancy. Stop taking olmesartan medoxomil as soon as you find out you are pregnant. Some people, especially those who are low on body fluids or salt, may experience low blood pressure. Rarely, this drug can cause low urine output, build-up of waste in the blood, and sudden kidney failure. People with narrowed arteries in their kidneys are at greater risk for severe low blood pressure and kidney problems. If you have kidney issues, use this medicine carefully and check your blood’s creatinine and potassium levels regularly. Do not use if you have severe kidney (CrCl< 20 mL/min) or liver problems. Be careful if you have heart valve problems or a thickened heart muscle. This medicine might not work well for people with a condition called primary aldosteronism. It is also not recommended while breastfeeding.

Drug Interactions
There is a higher chance of developing high potassium levels (hyperkalaemia) if this medication is taken with ACE inhibitors, potassium-sparing diuretics, potassium supplements, or other drugs that raise potassium (like ciclosporin or eplerenone). Taking it with other blood pressure medications can make your blood pressure drop even more. Using it with NSAIDs may lower kidney function and could cause sudden kidney failure. It can also increase lithium levels in your blood, raising the risk of lithium toxicity.
Contraindications
Pregnancy (2nd and 3rd trimesters); biliary obstruction.
Mode of actions
Olmesartan is a medicine that works by blocking a specific receptor (AT1) for a hormone called angiotensin II. This hormone normally causes blood vessels to tighten and makes the body release another hormone called aldosterone. By blocking these effects, olmesartan helps relax and widen the blood vessels, which lowers blood pressure and allows more blood and oxygen to reach the heart.
Dosage & Administration
Oral
Hypertension
Adult: Initial: 10-20 mg once daily may then be increased up to max 40 mg once daily if needed.
Child: 6-16 yr 35 kg: 20 mg once daily. Doses may be doubled once if necessary after 2 wk.
Elderly: No dosage adjustment needed.
Hepatic impairment: Moderate: Initial: 10 mg once daily may increase up to max 20 mg once daily.
Pregnancy & Lactation
This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.
There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
