High doses can be dangerous for people with poor kidney function (risk of nerve damage) or heart failure. Avoid direct contact with the drug, as it can cause skin allergies. Regularly check blood potassium levels, kidney function, and blood cell counts. Used for spirochete infections like syphilis; using for a long time may lead to secondary infections with penicillin-resistant bacteria. Do not give by intrathecal (spinal) injection. It may also cause damage to the esophagus.
Probenecid makes pivmecillinam stay in your body longer. Taking it with blood thinners can cause you to bleed for longer than usual. It can also make birth control pills less effective. If you use it with valproate, you may have a higher risk of low carnitine levels.
Pivmecillinam is converted in the body to mecillinam, which fights bacteria by blocking the formation of their cell walls. It works well against many types of gram-negative bacteria, especially those in the Enterobacteriaceae family, like Escherichia coli, Enterobacter, Klebsiella, Salmonella, and Shigella species. However, bacteria such as indole-positive Proteus and Serratia marcescens are resistant to it. Pivmecillinam can be broken down by beta-lactamase enzymes, but it is more stable against these enzymes than ampicillin.

Adult: Initially 400 mg followed by 200 mg tid for 8 doses.

Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
