Zoledron is indicated for

  • Hypercalcaemia of malignancy

Precautions and warnings

Patients should be well-hydrated before getting Zolenic, especially older adults and those taking diuretics. To stay hydrated, patients should drink two glasses of water (or other fluids) before and after the infusion. If a patient has low calcium levels, this should be corrected first by taking enough calcium and vitamin D before starting Zolenic. Any other mineral imbalances, such as those caused by issues with the parathyroid or thyroid glands, or problems absorbing calcium in the intestines, should also be treated beforehand. Doctors should carefully monitor these patients during treatment.

Side-effects

Drug Interactions

There is a higher risk of low calcium levels (hypocalcaemia) when aminoglycosides or loop diuretics are used. There is also a greater chance of kidney problems when taken with medicines that are harmful to the kidneys (nephrotoxic agents).

Contraindications

Do not use this drug if you are allergic to the main ingredient, any of its other components, or any bisphosphonates. It should also not be used if you have low levels of calcium in your blood or if you have kidney problems (such as reduced kidney function).

Mode of actions

Zoledronic acid is a type of medicine called an aminobiphosphonate. It effectively slows down the breakdown of bone by blocking the actions of cells called osteoclasts. This helps prevent bones from losing calcium, especially when that loss is caused by tumors.

Dosage & Administration

Intravenous
Hypercalcaemia of malignancy
Adult: 4 mg as a single dose by infusion over at least 15 minutes. Retreatment in patients who relapse or who are refractory to initial treatment: 4 mg as an infusion over at least 15 minutes, after at least 1 wk from initial dose.


Bone metastases associated with solid tumours
Adult: 4 mg infusion over at least 15 minutes every 3-4 wk, in conjunction with 500 mg calcium and 400 IU vitamin D oral supplement.

50-60 3.5 mg every 3-4 wk
40-49 3.3 mg every 3-4 wk
30-39 3 mg every 3-4 wk

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
