Precautions and warnings

Lactic Acidosis and Severe Liver Enlargement with Fat Buildup:
There is a risk of these serious liver problems.
Use caution in people with liver or kidney problems.
Do not use if kidney function (CrCl) is very low.

Drug Interactions

Atazanavir levels become lower when taken with tenofovir, unless ritonavir is also given. When tenofovir or emtricitabine is taken with other drugs that are removed from the body through the kidneys, the levels of both drugs can go up.

**Potentially Dangerous Effects:**
- There is a higher risk of kidney damage if these drugs are used recently or at the same time with other kidney-damaging medicines (such as aminoglycosides, amphotericin B, foscarnet, ganciclovir, pentamidine, vancomycin, cidofovir, or interleukin-2). If these combinations can't be avoided, kidney function should be checked every week.
- Taking didanosine together will increase its level in the blood, raising the risk of pancreatitis and nerve problems, and causing treatment to fail—so these drugs should not be used together.
- Do not use emtricitabine with lamivudine, because they have similar resistance patterns.
- Using ?-interferon together raises the risk of lactic acidosis.

Contraindications

Do not use while breastfeeding. This should not be used to treat long-term hepatitis B virus (HBV) infection.

Mode of actions

Tenofovir disoproxil fumarate is a prodrug, meaning it is changed in the body into its active form, tenofovir. Tenofovir works as a nucleotide analogue, which means it mimics part of the DNA building blocks. Emtricitabine is a man-made drug that acts like another DNA building block called cytidine. Both drugs block the HIV-1 reverse transcriptase enzyme, which is needed for the virus to make copies of itself. This action stops the virus's DNA from forming properly, which helps prevent HIV from multiplying.

Dosage & Administration

Adult: One tablet once daily taken orally with or without food.

Renal impairment: Patient on haemodialysis: Not recommended.
CrCl (ml/min)
30-49 1 tablet every 48 hr.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
