Precautions and warnings

Certain conditions can get worse due to fluid retention, such as high calcium levels in the blood, stroke, heart disease, and gallbladder problems. There may also be effects on blood fats, especially in people with inherited issues in fat metabolism. This medication can raise blood pressure and increase the risk of blood clots, breast cancer, benign liver tumors, and endometrial (uterine lining) cancer, as well as make existing uterine fibroids larger. If you have liver problems, the dose should be lowered. Caution is advised when using this medicine during breastfeeding or in children.

Drug Interactions

Drugs that increase the activity of CYP1A2 and CYP3A4 enzymes, such as aminoglutethimide, carbamazepine, phenobarbital, and rifampin, can lower the effectiveness of estradiol. When taken with hydrocortisone or prednisolone, these drugs may make their effects stronger.

Contraindications

Do not use if you have: an allergy to this medicine; unexplained vaginal bleeding; blood clots or clotting disorders; breast cancer (unless being treated specifically for advanced disease); estrogen-dependent tumors; porphyria; or if you are pregnant.

Mode of actions

Estradiol is a natural form of estrogen, a hormone that helps develop and maintain the female reproductive system and features like breast growth and body shape. Estrogens also control the release of other hormones (LH and FSH) from the pituitary gland by signaling it to produce less when there’s enough estrogen in the body.

Dosage & Administration

Oral
Moderate to severe vasomotor symptoms associated with menopause
Adult: 1-2 mg/day adjusted as necessary; admin can be cyclical (3 wk on, 1 wk off) or continuous. In conjunction with a progestogen in women with uterus.

Prophylaxis of osteoporosis in postmenopausal women
Adult: 0.5 mg/day in cyclic regimen (23 days on and 5 days off).

Hypogonadism
Adult: 1-2 mg/day in a cyclic regimen for 3 wk on drug, followed by 1 wk drug-free.

Vaginal
Atrophic vaginitis
Adult: Initial: Insert 1 tab (20 mcg) once daily for 2 wk. Maintenance: Insert 1 tab twice wkly. Attempt to discontinue or taper medication at 3-6 mthly intervals.

Pregnancy & Lactation

This medicine must not be used during pregnancy. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict two effective types of birth control starting one month before and during treatment and for three years after stopping this medicine. Discuss with your doctors if you are not sure which types of birth control are effective for you. You must need to take pregnancy tests regularly during your treatment with this medicine and for at least three years after taking this medicine. If it seems that you do become pregnant stop taking this medicine and call your doctor immediately. If you really become pregnant, miss a menstrual period, or have sex without using two forms of birth control. In some cases, your doctor can prescribe emergency contraception to prevent pregnancy. People on this medicine should not donate blood during the treatment and for three years afterward.

This medicine is also contraindicated breast milk. Breastfeeding is not recommended while using this medication and for three years afterward.
