Precautions and warnings
Use caution when giving citalopram to patients with epilepsy, those receiving electroconvulsive therapy, a history of mania, heart disease, diabetes, angle-closure glaucoma, bleeding disorders, liver or kidney problems, or during pregnancy. Do not stop citalopram suddenly—reduce the dose slowly, especially if the patient becomes manic. Citalopram can increase the risk of low sodium levels (hyponatraemia) and SIADH. It might also lower the seizure threshold, so extra care is needed for people with epilepsy.

Drug Interactions
This drug can increase the effect of blood thinners like warfarin, raising the risk of bleeding. Taking it with sibutramine may raise the chance of having hypomania. It can also make it easier to have seizures if taken with TCAs or other SSRIs.
Serious Risks: Taking this drug with MAOIs can cause dangerous side effects like serotonin syndrome. It can also increase the risk of heart rhythm problems (QT prolongation and torsades de pointes) if taken with other drugs that affect the heart's rhythm, such as pimozide, quinidine, procainamide, chlorpromazine, thioridazine, amiodarone, sotalol, moxifloxacin, pentamidine, levomethadyl, or methadone.
Contraindications
Do not use if you are allergic to this medication, if you are taking MAOIs, or if you have stopped taking MAOIs within the last 14 days. This medication should also not be given to children or teenagers.
Mode of actions
Citalopram is a type of antidepressant known as a selective serotonin reuptake inhibitor (SSRI). It mainly works by increasing the amount of serotonin in the brain, without significantly affecting other chemicals like noradrenaline, dopamine, or GABA. This action is what makes citalopram effective in treating depression. Citalopram doesn’t strongly interact with many other receptors in the brain, such as those for histamine, dopamine, adrenaline, acetylcholine, or opioids, which helps limit its side effects.
Dosage & Administration
Adult: Initially, 20 mg daily, increased to 40 mg once daily after at least 1 wk or 60 mg daily if necessary.
Hepatic impairment: Recommended dose: 20 mg daily.
Oral Depressive phase of bipolar disorder: Adult: Initially, 20 mg daily, increased to 40 mg once daily after at least 1 wk or 60 mg daily if necessary.
Hepatic impairment: Recommended dose: 20 mg daily.
Oral Panic disorder with or without agoraphobia: Adult: Initially, 10 mg daily, increased to 20 mg daily after 1 wk. Max: 60 mg daily.
Hepatic impairment: Recommended dose: 20 mg daily. Special Populations: Patients with reduced hepatic function should receive dosages of no more than 30 mg/day.
Elderly maximum 40 mg daily.
Pregnancy & Lactation
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
