Precautions and warnings

Patients with congestive heart failure (CHF), high blood pressure, diabetes, epilepsy, or elderly people, as well as those on long-term medication, need special care. Stopping treatment should be done gradually. Extra caution is also needed during pregnancy and breastfeeding.

Drug Interactions

Thiazides can make the high blood sugar and low potassium caused by corticosteroids worse. Taking NSAIDs at the same time can increase the risk of stomach ulcers or bleeding. Corticosteroids can also change how your body responds to blood thinners. People taking antidiabetic or blood pressure medicines might need higher doses. Corticosteroids can lower the amount of salicylates and antimuscarinic drugs in your blood. Drinking alcohol may increase stomach irritation.

The effects of corticosteroids are reduced if taken with certain drugs like carbamazepine, phenytoin, primidone, barbiturates, or rifampicin. When taken with ciclosporin, both drugs can raise each other’s levels in the blood. The effect of corticosteroids is stronger in women taking estrogen or birth control pills.

Contraindications

Do not use for viral or fungal infections, tuberculosis or syphilis lesions, or bacterial infections unless given together with the proper antibiotics.

Mode of actions

Hydrocortisone is a corticosteroid medicine that helps reduce inflammation and lowers the immune system’s response. It works by stopping certain white blood cells (called polymorphonuclear leukocytes) from moving to areas of swelling and by making blood vessels less leaky. Hydrocortisone can also be used to replace hormones when the body doesn’t make enough due to problems with the adrenal glands.

Dosage & Administration

Adult: By IM injection or slow IV injection or infusion, 100-500 mg, 3-4 times in 24 hours or as required;
Child: by slow IV Injection, up to 1 year: 25 mg, 1-5 years: 50 mg, 6-12 years: 100 mg.

PO Adrenocortical insufficiency 20-30 mg/day in 2 divided doses.

IV Supplement in adrenal insufficiency during surgery under general anesth Patients taking >10 mg/day prednisolone or its equivalent by mouth: 25-50 mg at induction.

Acute adrenocortical insufficiency 100-500 mg 3-4 times/24 hr depending on severity and response.

Inj Soft-tissue inflammation As Na phosphate or Na succinate esters: 100-200 mg as local inj.

Intra-articular Joint inflammations As acetate: 5-50 mg depending on size of affected joint.

Children: By slow IV injection, up to 1 year 25 mg, 1-5 years 50 mg, 6-12 years 100 mg.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
