Fixocard 5/25 is indicated for

  • Patients with essential hypertension; Patients with angina pectoris & hypertension as co-existing diseases; In post MI patients; In patients with refractory angina pectoris where nitrate therapy has failed.

Precautions and warnings

In senior people, a rapid drop in blood pressure is possible. Patients with COPD, thyrotoxicosis, congestive failure, vasospastic angina, hepatic, renal impairment should exercise caution. Beta-blockers should be used with caution in diabetic individuals because they can conceal tachycardia caused by hypoglycemia. Withdrawal should be done in stages. Lactation. Children's safety and efficacy are yet to be determined. In untreated phaeochromocytoma, do not use.

Drug Interactions

When used with catecholamine-depleting medicines, there is a risk of hypotension and/or significant bradycardia. To avoid rebound hypertension, clonidine should be terminated a few days after the beta-blocker has been stopped; if replacing clonidine with beta-blocker, beta-blocker should be started only after clonidine has been stopped for several days. Concurrent use with prostaglandin synthase inhibitors (such as indomethacin) may diminish beta-blocker hypotensive effects.

Contraindications

Hypotension, sinus bradycardia, 2nd, 3rd degrees of heart block, cardiogenic shock, overt congestive failure, poor LV function, hypersensitivity to either component, and pregnancy are all factors to consider.

Mode of actions

Atenolol is a beta-blocker that is cardioselective. Amlodipine is a dihydropyridine calcium-channel blocker that prevents calcium ions from crossing the blood-brain barrier and entering vascular smooth muscle and cardiac muscle. When the two medications are combined, they have an additive antihypertensive effect.

Dosage & Administration

Adult: PO HTN; Chronic stable angina Per tab contains amlodipine (besylate) 5 mg and atenolol 25 or 50 mg: 1 tab once daily, up to 2 tab/day if needed.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Another study shows that this medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
