Hartmann IV is indicated for

Precautions and warnings

It should not be given in large doses or for long periods of time. Because the solution contains a variety of electrolytes, it should be used with caution in patients who are at risk for electrolyte imbalance, such as pregnant women, renal impairment, heart failure, pulmonary congestion, and patients using potassium-sparing diuretics.

If the residual air in the container is not thoroughly evacuated before to administration, pressurizing intravenous solutions stored in flexible plastic containers to enhance flow rates can result in air embolism.

Air embolism could occur if a vented intravenous administration device is used with the vent in the open position. Flexible plastic containers should not be used with vented IV administration systems with the vent open.

During prolonged parenteral therapy or anytime the patient's condition merits it, clinical examination and periodic laboratory determinations are required to monitor changes in fluid balance, electrolyte concentrations, and acid-base balance.

Ringer's Injection, USP should be used with caution in individuals who are taking corticosteroids or corticotropin.

Side-effects

  • Reactions which may occur because of the solution or the technique of administration include febrile response
  • infection at the site of injection
  • venous thrombosis or phlebitis extending from the site of injection
  • extravasation
  • and hypervolemia. If an adverse reaction does occur
  • discontinue the infusion
  • evaluate the patient
  • institute appropriate therapeutic countermeasures
  • and save the remainder of the fluid for examination if deemed necessary.

Contraindications

Hypersensitivity to the active ingredient.

Mode of actions

Calcium chloride is used to treat or prevent calcium deficiency. It also helps nerve and muscle performance by regulating the action potential excitation threshold.

The intracellular fluid contains potassium chloride, which is a significant cation. It is involved in nerve impulse conduction in the heart, brain, and skeletal muscle, cardiac skeletal and smooth muscle contraction, normal renal function, acid-base balance, glucose metabolism, and gastric secretion.

The most abundant extracellular cation is sodium chloride. It restores sodium ions, which is crucial for electrolyte and fluid balance, osmotic pressure management, and water distribution. It is used as a source of electrolytes and water for hydration, treatment of metabolic acidosis, priming solution in hemodialysis and treatment of hyperosmolar diabetes. It is also used as diluents for the infusion of compatible drug additives.

Sodium lactate is a nonpyrogenic, sterile solution that is used to restore fluids and electrolytes. It has metabolic alkalinizing properties and can induce diuresis depending on the patient's clinical status.

Dosage & Administration

As directed by a physician. Dosage is dependent upon the age, weight and clinical condition of the patient as well as laboratory determinations.

The recommended flow rate is up to 100-drops/minute/70 kg body weight. In burn patients the dose is according to the Parkland formula : 4ml/kg body weight/% of Body surface area (BSA) burn (e.g. for a 30% BSA burn of a person having 60 kg body weight, 4 x 60x30ml = 7200 ml of Lactoride would be required in 24 hours). Half of this within 8 hr, the remainder over 16 hr.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
