Precautions and warnings
Seien Sie vorsichtig bei Menschen mit Epilepsie, älteren Menschen und Personen, die Aufgaben ausführen, die geistige Wachsamkeit erfordern. Seien Sie auch vorsichtig bei Erkrankungen wie Engwinkelglaukom, Verstopfung im Magen oder Darm, Verstopfung der Harnwege, Überfunktion der Schilddrüse (Hyperthyreose), erhöhtem Augeninnendruck, Herzerkrankungen, schwerem Asthma und während der Schwangerschaft.

Drug Interactions
Maskiert die schädlichen Wirkungen auf das Ohr (Ototoxizität), die durch Aminoglykosid-Antibiotika verursacht werden. Verringert die Wirksamkeit von Levodopa, da es im Magen schneller abgebaut wird und seine Absorption durch eine Verzögerung der Magenentleerung verlangsamt wird. Reduziert die Wirksamkeit cholinerger Medikamente wie Tacrin und Donepezil sowie bestimmter Antipsychotika. Pflanzliche Nahrungsergänzungsmittel wie Baldrian, Johanniskraut, Kava Kava und Gotu Kola können Schläfrigkeit oder Beruhigung verstärken.
**Gefahr:**
Das Mischen mit Alkohol, Barbituraten, Schmerzmitteln, Schlaftabletten und Antipsychotika kann die Schläfrigkeit oder Depression des Zentralnervensystems gefährlich verstärken. Wenn es zusammen mit MAO-Hemmern, Atropin oder trizyklischen Antidepressiva (TCAs) eingenommen wird, kann es auch die antimuskarinischen Nebenwirkungen verstärken.
Contraindications
Überempfindlichkeit; Neugeborene, Stillzeit.
Mode of actions
Diphenhydramin wirkt, indem es H1-Histaminrezeptoren im Verdauungssystem, in den Blutgefäßen und in den Atemwegen blockiert. Es kann zu Schläfrigkeit führen und hat auch ähnliche Wirkungen wie Arzneimittel, die Acetylcholin blockieren (anticholinerge Wirkung).
Dosage & Administration
Mündlich
Erwachsener: Als Hydrochlorid: 25-50 mg 3-4 mal täglich. Maximal: 300 mg/Tag.
Kind: 6,25–25 mg 3–4 mal täglich, bis zu 5 mg/kg in mehreren Dosen. Maximal: 300 mg/Tag.
Behandlung und Prophylaxe der Reisekrankheit
Erwachsener: Als Diphenhydramindi (Acefyllinat): Übliche Dosis: 90–135 mg, kann bei Bedarf in Abständen von mindestens 6 Stunden wiederholt werden. Maximal: 540 mg täglich. Zur Vorbeugung sollte die Dosis mindestens 30 Minuten vor Reiseantritt verabreicht werden.
Pregnancy & Lactation
Obwohl tierexperimentelle Reproduktionsstudien eine nachteilige Wirkung gezeigt haben und manchmal sogar kein Risiko für den Fötus nachgewiesen werden konnte, und es keine ausreichenden und gut kontrollierten Studien bei schwangeren Frauen gibt, kann dieses Arzneimittel während der Schwangerschaft nur dann angewendet werden, wenn dies unbedingt erforderlich ist.
