Precautions and warnings
लीवर या किडनी की समस्याएँ, या कमज़ोर अस्थि मज्जा। इस दवा का उपयोग मेस्ना के साथ करें और मूत्राशय के दुष्प्रभावों को रोकने में मदद के लिए बहुत सारे तरल पदार्थ (मुंह से या IV) पीना सुनिश्चित करें।

Drug Interactions
एलोप्यूरिनॉल या सिस्प्लैटिन के साथ लेने पर इफोसफामाइड अधिक विषैला हो सकता है। यह वारफारिन को और अधिक मजबूती से काम करने पर मजबूर कर सकता है, जिससे इसके रक्त-पतला प्रभाव में वृद्धि हो सकती है। कुछ दवाएं जो CYP2A6 एंजाइम को तेज़ करती हैं, जैसे कि अमोबार्बिटल, फेनोबार्बिटल, पेंटोबार्बिटल, रिफैम्पिन और सेकोबार्बिटल, आपके रक्त में इफोसफामाइड की मात्रा को कम कर सकती हैं। इसके विपरीत, ऐसी दवाएं जो इस एंजाइम को धीमा कर देती हैं, जैसे आइसोनियाज़िड, मेथॉक्ससेलेन और माइक्रोनाज़ोल, इफोसफामाइड के स्तर को बढ़ा सकती हैं।
इसी तरह, दवाएं जो CYP3A4 गतिविधि को बढ़ाती हैं - जैसे कि एमिनोग्लुटेथिमाइड, कार्बामाज़ेपाइन, नेफसिलिन, नेविरापीन, फ़ेनोबार्बिटल, फ़िनाइटोइन और रिफ़ामाइसिन - भी रक्त में इफोसफ़ामाइड के स्तर को कम कर सकती हैं। दूसरी ओर, दवाएं जो CYP3A4 को अवरुद्ध करती हैं - जैसे एज़ोल एंटीफंगल, क्लैरिथ्रोमाइसिन, डाइक्लोफेनाक, डॉक्सीसाइक्लिन, एरिथ्रोमाइसिन, इमैटिनिब और आइसोनियाज़िड - इफोसफामाइड के स्तर को बढ़ा सकती हैं।

अतिसंवेदनशीलता; गंभीर अस्थि-मज्जा अवसाद. गर्भावस्था, स्तनपान.
Mode of actions
इफोसफामाइड को लीवर एंजाइम द्वारा सक्रिय रूपों में बदल दिया जाता है जो एल्काइलेटिंग एजेंट के रूप में काम करता है, जो लक्ष्य कोशिकाओं में डीएनए और प्रोटीन उत्पादन में हस्तक्षेप करता है। मूत्राशय और मूत्र पथ को होने वाले नुकसान को रोकने में मदद के लिए, इसे आमतौर पर मेस्ना के साथ दिया जाता है।
Dosage & Administration
वयस्क: IV लिंफोमा; सारकोमा; ठोस ट्यूमर विभिन्न लाइसेंस प्राप्त खुराक आहार उपलब्ध हैं। 8-12 ग्राम/एम2 को 3-5 दिनों में विभाजित करें, पाठ्यक्रम 2-4 सप्ताह में दोहराएं।
जर्म सेल टेस्टिकुलर कार्सिनोमा डब्ल्यू/मेस्ना और पर्याप्त जलयोजन: 5 दिनों के लिए 1.2 ग्राम/एम2/दिन, सप्ताह में 3 बार उपचार दोहराएं या हेमेटोलॉजिकल विषाक्तता से ठीक होने के बाद।

Pregnancy & Lactation
गर्भावस्था के दौरान इस दवा की अनुमति नहीं है। यदि आप गर्भवती हैं तो इसका प्रयोग न करें। यह त्वचा और फेफड़ों के माध्यम से अवशोषित हो सकता है, जिससे आपके अजन्मे बच्चे के अंगों को नुकसान हो सकता है और जन्मजात विकलांगता हो सकती है। उपचार के दौरान और इस दवा को रोकने के छह महीने बाद तक सख्त प्रभावी जन्म नियंत्रण। यदि आप गर्भवती हो जाती हैं, तो इस दवा को लेना बंद कर दें और तुरंत अपने डॉक्टर से चर्चा करें।
यह दवा स्तन के दूध में पारित होती है या नहीं, इस पर कोई विशेष जानकारी नहीं है। बच्चों के लिए संभावित जोखिमों को ध्यान में रखते हुए, इस दवा का उपयोग करते समय स्तनपान कराने की सिफारिश नहीं की जाती है और आखिरी खुराक के कम से कम दो सप्ताह बाद स्तनपान कराने की सलाह दी जाती है। स्तनपान कराने से पहले अपने डॉक्टर से परामर्श लें।
