Precautions and warnings
توخي الحذر عند الأشخاص المصابين بالصرع، وكبار السن، وأولئك الذين يقومون بمهام تحتاج إلى اليقظة العقلية. كن حذرًا أيضًا في حالات مثل زرق انسداد الزاوية، وانسداد المعدة أو الأمعاء، وانسداد المسالك البولية، وفرط نشاط الغدة الدرقية (فرط نشاط الغدة الدرقية)، وزيادة الضغط داخل العين، وأمراض القلب، والربو الحاد، وأثناء الحمل.
Side-effects
- اكتئاب الجهاز العصبي المركزي، والدوخة، والصداع، والتخدير. التحفيز المتناقض عند الأطفال. جفاف الفم، زيادة إفرازات الجهاز التنفسي، عدم وضوح الرؤية، احتباس البول. اضطرابات الجهاز الهضمي. خلل التنسج الدموي.
Drug Interactions
يخفي آثار الأذن الضارة (التسمم الأذني) التي تسببها المضادات الحيوية أمينوغليكوزيد. يجعل الليفودوبا أقل فعالية عن طريق تكسيره بشكل أسرع في المعدة وإبطاء امتصاصه عن طريق تأخير إفراغ المعدة. يقلل من فعالية الأدوية الكولينية، مثل التاكرين والدونيبيزيل، بالإضافة إلى بعض مضادات الذهان. المكملات العشبية مثل حشيشة الهر، ونبتة سانت جون، والكافا كافا، وغوتو كولا يمكن أن تزيد من النعاس أو التخدير.
**خطر:**
يمكن أن يؤدي الخلط مع الكحول والباربيتورات ومسكنات الألم والحبوب المنومة ومضادات الذهان إلى زيادة النعاس أو الاكتئاب في الجهاز العصبي المركزي بشكل خطير. عند تناوله مع مثبطات MAOI، أو الأتروبين، أو مضادات الاكتئاب ثلاثية الحلقات (TCAs)، فإنه يمكن أيضًا أن يزيد من الآثار الجانبية المضادة للمسكارين.
Contraindications
فرط الحساسية ; حديثي الولادة، الرضاعة.
Mode of actions
يعمل ديفينهيدرامين عن طريق منع مستقبلات الهيستامين H1 الموجودة في الجهاز الهضمي والأوعية الدموية والممرات الهوائية. يمكن أن يجعلك تشعر بالنعاس وله أيضًا بعض التأثيرات المشابهة للأدوية التي تمنع الأسيتيل كولين (تأثيرات مضادات الكولين).
Dosage & Administration
عن طريق الفم
للبالغين: على شكل هيدروكلوريد: 25-50 مجم 3-4 مرات يومياً. الحد الأقصى: 300 ملغ/يوم.
الأطفال: 6.25-25 مجم 3-4 مرات يومياً، حتى 5 مجم/كجم مقسمة على جرعات. الحد الأقصى: 300 ملغ/يوم.
العلاج والوقاية من دوار الحركة
للبالغين: مثل ديفينهيدرامين دي (أسيفيلينات): الجرعة المعتادة: 90-135 مجم، يمكن تكرارها إذا لزم الأمر على فترات لا تقل عن 6 ساعات. الحد الأقصى: 540 ملغ يومياً. للوقاية، يجب إعطاء الجرعة قبل السفر بـ 30 دقيقة على الأقل.
Pregnancy & Lactation
على الرغم من أن دراسات التكاثر الحيواني أظهرت تأثيرًا سلبيًا حتى في بعض الأحيان غير قادرة على إثبات المخاطر على الجنين ولا توجد دراسات كافية ومراقبة بشكل جيد عند النساء الحوامل، ولكن أثناء الحمل، يمكن استخدام هذا الدواء فقط عند الحاجة إليه بالتأكيد.
