Precautions and warnings

People with kidney or liver problems, psychotic disorders, serious heart disease, or who are pregnant should be careful with this medication. Suddenly stopping the medicine can cause a serious condition called neuroleptic malignant syndrome. Some patients may have troubling drops in blood pressure when starting treatment. Regular eye check-ups are advised because vision problems may occur. This medicine may make it unsafe to drive or use machines. There is also a risk of muscle breakdown (rhabdomyolysis) when using this drug.

Drug Interactions

It can block the kidney’s active removal (secretion) of certain basic drugs, like cimetidine. This means drugs that depend on this process to leave the body—such as amantadine—may stay in your system longer, so their clearance is reduced. It can also increase the effects of alcohol and other sedatives.

Mode of actions

Pramipexole is a type of medicine that helps treat the movement problems seen in Parkinson’s disease. It works by directly increasing dopamine activity in parts of the brain called the striatum and substantia nigra. Pramipexole can be used together with levodopa to help control symptoms in people with advanced Parkinson’s disease, or it can be used alone to manage symptoms in the early stages of the condition.

Dosage & Administration

Adult: PO Parkinson's disease As adjunct to levodopa therapy: Initial: 125 mcg 3 times/day in the 1st wk, increase to 250 mcg 3 times/day in the 2nd wk and 500 mcg 3 times/day in the 3rd wk according to response. May further increase if needed. Max: 4.5 mg/day.

Renal impairment:
CrCl (ml/min)
20-50 125 mcg bid. Max: 2.25 mg/day.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
