People with liver disease, a history or risk of blood clots, high blood sugar, high cholesterol or triglycerides, high blood pressure, gallstones, lupus (SLE), uterine fibroids, endometriosis, or a history of thickening of the womb lining should be cautious. Conditions that can get worse with fluid retention—such as kidney problems, migraines, or epilepsy—should also be considered. Stop treatment if you develop blood clots, abnormal liver tests, a significant rise in blood pressure, or new migraine-like headaches. This medication is not recommended for women in the first year after menopause due to the chance of irregular vaginal bleeding. Stop taking tibolone four weeks before planned surgery, especially if you will need to be inactive for a long time afterward. You may also need to change your diabetes medication.
Drugs called enzyme inducers—such as barbiturates, phenytoin, carbamazepine, and rifampicin—can speed up how quickly tibolone is broken down in the body. Tibolone may also increase the blood-thinning effects of warfarin.
Women should not use this medication if they have or may have estrogen-dependent tumors, have or have had breast cancer, unexplained vaginal bleeding, severe liver problems, a history or current heart or blood vessel disease, untreated endometrial hyperplasia, porphyria, are pregnant or breastfeeding, or are premenopausal.
Tibolone is a type of steroid that acts like estrogen and progesterone, with some mild effects similar to androgens.
Adult: PO Climacteric symptoms & other post-menopausal problems in adults and elderly women: The dosage is 2.5 mg tablet per day, preferably at the same time of the day. Symptomatic relief occurs within few days, but optimal results are obtained minimum after 3 months.
Osteoporosis: To prevent osteoporosis in post-menopausal women or after oophorectomy an uninterrupted longer (5-10 years) therapy of 2.5 mg daily is needed.
Starting Tibolone:
In case of natural menopause: To avoid irregular & abnormal bleeding, Tibolone should be started at least 12 months after last natural bleeding.
In case of artificial menopause: Treatment should be started immediately.
In case of switching over from other conventional HRT: If someone switches from estrogen-only preparation, she should start after a withdrawal bleeding. If someone switches from sequential HRT preparation she should wait for the completion of the progestogen phase. If someone switches from continuous combined HRT, she can start any time.
In case of missed pill: If no more than 12 hours have passed, the pill should be taken immediately. Otherwise the next dose should be continued as before.
This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.
There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
