Use this medication only if migraine has been clearly diagnosed and other serious neurological conditions have been ruled out. Check blood pressure regularly. This medication may make it unsafe to drive or use machines. Use with caution during pregnancy, breastfeeding, in children, and in people with moderate to severe liver problems. People with risks for heart disease should have a heart check before starting; their first dose should be given under medical supervision with an ECG if the checkup is good. Patients with heart risk factors taking zolmitriptan should have regular heart checkups. Patients with phenylketonuria should not take the orally disintegrating tablet form. Using zolmitriptan long-term may cause it to build up in melanin-rich tissues, such as the eyes.
**There is a higher risk of serotonin syndrome when zolmitriptan is taken with SSRIs (selective serotonin reuptake inhibitors) or SNRIs (serotonin and norepinephrine reuptake inhibitors). The amount of zolmitriptan in your body can also increase if you take it with cimetidine, certain antibiotics called quinolones, or other drugs that block the CYP1A2 enzyme.**
**Serious Risk:** Taking zolmitriptan with sibutramine can cause deadly serotonin syndrome. The risk of serotonin syndrome is also higher if you use MAO inhibitors (MAOI) or similar medicines called RIMAs. Do not use zolmitriptan while taking these drugs or for 2 weeks after stopping them. Also, do not use zolmitriptan within 24 hours of taking another 5-HT1 agonist. Using zolmitriptan with ergot alkaloids (used for migraines) can cause dangerous narrowing of blood vessels, so you should not take zolmitriptan within 24 hours of any ergotamine or similar medication.
People who are at risk for coronary artery disease include post-menopausal women, men over 40, and those with high blood pressure, high cholesterol, those who smoke, are overweight, have diabetes, or have a family history of heart disease. These patients should be carefully checked. Other cases needing caution include those with Wolff-Parkinson-White syndrome, abnormal heart rhythms caused by extra electrical pathways in the heart, a history of stroke, uncontrolled or severe high blood pressure, heart disease caused by poor blood flow, history of heart attack, coronary artery spasms, or brief episodes of reduced blood flow to the brain (TIAs). Patients with basilar migraine or hemiplegic migraine should also be assessed carefully.
Zolmitriptan works by specifically targeting serotonin (5HT1) receptors. It helps treat migraines by narrowing certain blood vessels in the brain, reducing the release of pain-causing chemicals, and lowering the pain signals along the trigeminal nerve pathway.
Oral
Acute migraine attacks
Adult: Initially, 2.5 mg. A 2nd dose may be taken at least 2 hr after the 1st dose if symptoms recur within 24 hr. Max dose: 10 mg in 24 hr. Safety of treating > three headaches in a 30-day period has not been established.
Hepatic impairment: Dose reduction may be needed with BP monitoring. Moderate to severe: Max dose: 5 mg/24 hr
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
