Be cautious when using gentamicin at the same time as neuromuscular blocking agents or in people with myasthenia gravis, Parkinson’s disease, or those at risk for ear and kidney damage. Avoid during breastfeeding. Check gentamicin levels in the blood for patients taking high doses or longer treatments—especially infants, elderly people, those with kidney problems, cystic fibrosis, or significant obesity. Regularly monitor hearing and kidney function during treatment.
**Gentamicin:**
Taking gentamicin with other drugs that can harm the nerves or kidneys — such as cephalosporins, methicillin, amphotericin B, ciclosporin, cisplatin, strong diuretics (like ethacrynic acid or furosemide), or muscle relaxants (e.g., succinylcholine, tubocurarine) — can increase the risk of side effects. Gentamicin may make blood thinners (e.g., warfarin, phenindione) work more strongly. It can reduce the effect of medicines like neostigmine and pyridostigmine. The risk of low blood calcium is higher if taken with bisphosphonates, and the chance of muscle weakness increases if taken with botulinum toxin. In newborns, indometacin can cause higher levels of gentamicin in the blood.
**Hydrocortisone:**
Thiazide diuretics can worsen high blood sugar and low blood potassium caused by hydrocortisone. Taking non-steroidal anti-inflammatory drugs (NSAIDs) at the same time increases the risk of stomach ulcers or bleeding. Hydrocortisone can change how anticoagulants work, and people may need to increase their doses of diabetes or blood pressure medicines. It reduces the levels of salicylates and antimuscarinic drugs in the blood, and drinking alcohol may make stomach irritation worse. Drugs like carbamazepine, phenytoin, primidone, barbiturates, and rifampicin can reduce how well hydrocortisone works. When taken with ciclosporin, both drugs raise each other's levels in the blood. Hydrocortisone may work more strongly in women taking estrogen or birth control pills.
Do not use if you have a history of allergic reactions to aminoglycosides, are pregnant, or have liver problems.
Gentamicin is an aminoglycoside antibiotic that works by attaching to the 30s and 50s parts of the ribosomes in bacteria. This blocks the bacteria from making proteins they need, which damages their cell membrane and eventually kills them.
Hydrocortisone is a corticosteroid drug used because it reduces inflammation and suppresses the immune system. It does this by preventing certain white blood cells from moving to inflamed areas and by making blood vessels less leaky. Hydrocortisone can also be used to replace hormones in people who don’t make enough on their own, such as those with adrenocortical insufficiency.
