Precautions and warnings

Trifluoperazine should be used with caution in people with heart disease, epilepsy, angle-closure glaucoma, Parkinson’s disease, myasthenia gravis, benign prostatic hyperplasia (enlarged prostate), diabetes, or problems with their kidneys or liver. It should also be used carefully in elderly people and in situations where there is exposure to very hot or cold temperatures.

If you are having a myelography (a type of X-ray test), stop taking trifluoperazine at least 48 hours before the test. Do not start it again until at least 24 hours after the procedure. Do not use trifluoperazine to treat nausea and vomiting before or after a myelography, especially if metrizamide is used for the procedure. Trifluoperazine should also be avoided during pregnancy.

Drug Interactions

Using this medication with other drugs that slow down the central nervous system (like opiates, some painkillers, barbiturates, sleeping pills, general anesthetics, or alcohol) can make drowsiness and sedation worse. Taking it with drugs that have antimuscarinic effects (such as certain antidepressants like tricyclics, or drugs for Parkinson’s disease) can raise the chances of side effects. It may reduce the effectiveness of medicines that increase dopamine, such as levodopa. There’s also a higher risk of low blood pressure if taken with blood pressure medicines or trazodone. It can reverse the blood pressure-lowering effect of guanethidine. Taking it with lithium can increase the risk of serious side effects involving movement or the nervous system. Lastly, antacids might lower how well this medicine is absorbed.

Contraindications

Should not be used in people who already have central nervous system (CNS) depression or are in a coma, have bone marrow problems, blood disorders, liver disease, or are allergic to phenothiazines. Not recommended for those with tumors that depend on prolactin. Should not be used during the first trimester of pregnancy or while breastfeeding.

Mode of actions

Trifluoperazine works by blocking dopamine D2 receptors in the brain. It does not cause much drowsiness or anticholinergic side effects, but it can strongly cause movement problems (extrapyramidal effects) and helps prevent nausea and vomiting. This medication is mainly used to control very severe, agitated, or aggressive behavior, but it can also be used to treat anxiety that is not related to psychosis.

Dosage & Administration

Adult: PO Psychoses 2-5 mg twice daily, up to 15-20 mg/day. Severe or resistant: 40 mg/day.
Nausea and vomiting; Short-term management of anxiety 1-2 mg twice daily. Max: 6 mg/day.
For anxiety: Max duration: 12 wk.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
