Precautions and warnings

For elderly patients, regularly check liver and lung function if the treatment lasts a long time. Be extra careful if the person already has lung, liver, nerve, or allergy problems, or is at risk for nerve damage—such as those with kidney problems, anemia, diabetes, electrolyte imbalances, weakness, or vitamin B deficiency. Stop the medication if there are any signs of nerve damage. Use with caution if breastfeeding.

Drug Interactions

Probenecid or sulfinpyrazone can decrease how much of this drug is removed from the body. Magnesium trisilicate can make it harder for your body to absorb the drug. Quinolone antibiotics can work against the effects of this drug. Medicines like carbonic anhydrase inhibitors or anything that makes the urine less acidic can lower how well this drug works.

Contraindications

Severe kidney problems include not making urine at all (anuria), making very little urine (oliguria), or having very high levels of creatinine in the blood, and a greatly reduced creatinine clearance (CrCl).

Mode of actions

Nitrofurantoin works by blocking several enzyme systems, such as acetyl coenzyme A, which disrupts the bacteria’s cell metabolism and the creation of their cell wall. It is able to kill most bacteria that cause urinary tract infections, including both gram-positive and gram-negative types.

Dosage & Administration

Oral
Acute uncomplicated urinary tract infections
Adult: 50-100 mg 4 times daily for 7 days. Dual-release preparation: 100 mg bid.
Child: >3 mth and older children: 3 mg/kg daily in 4 divided doses.

Prophylaxis of uncomplicated urinary tract infections
Adult: 50-100 mg at bedtime.
Child: >3 mth and older children: 1 mg/kg once daily.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
