Precautions and warnings

People with epilepsy or a tendency to have seizures, those who have known or suspected problems with their central nervous system, kidney or liver issues, myasthenia gravis, or are at risk of developing secondary infections (superinfections), as well as children, should use caution.

Drug Interactions

Taking certain antiarrhythmic medicines from class IA (like quinidine, procainamide) or class III (like amiodarone, sotalol) at the same time may increase the risk of a dangerous heart rhythm (QT interval prolongation). Antacids that have magnesium, aluminum, or calcium can lower the amount of medicine in your blood and urine. When taken with aminoglycoside antibiotics (like amikacin, tobramycin), there may be a stronger antibacterial effect. Using corticosteroids at the same time can increase the risk of serious tendon problems. NSAIDs can increase the risk of nervous system side effects, such as seizures. Taking this drug with theophylline can cause higher and longer-lasting theophylline levels in your blood, increasing the risk of side effects from theophylline.

Contraindications

Do not use if you are allergic to quinolones, are pregnant or breastfeeding, have a prolonged QT interval, or have untreated low potassium levels (hypokalaemia).

Mode of actions

Ofloxacin is a type of fluoroquinolone antibiotic that works by blocking bacterial enzymes called topoisomerase IV and DNA gyrase, which are needed for bacteria to copy, repair, and produce their DNA. This makes it effective against many kinds of both gram-negative and gram-positive bacteria.

Dosage & Administration

Adult: PO Part of multidrug therapy in leprosy 400 mg/day or intermittently, depending on regimen. Uncomplicated gonorrhoea Single dose of 400 mg. Acute bacterial exacerbation of chronic bronchitis;

Community-acquired pneumonia; Uncomplicated skin infections 400 mg twice daily for 10 days. Pelvic inflammatory disease 400 mg twice daily for 14 days. Postexposure prophylaxis following exposure to inhalational anthrax 400 mg twice daily for 60 days.

Legionnaire's disease 400 mg twice daily for 2-3 wk. Mixed infection of the urethra and cervix due to C. trachomatis and Neisseria gonorrhoeae; Nongonococcal cervicitis; Urethritis due to Chlamydia trachomatis 200-300 mg twice daily for 7 days.

Uncomplicated cystitis 200 mg twice daily for 3-7 days. Duration depends on causative agent. Complicated UTI 200 mg twice daily for 10 days. Traveller's diarrhoea 300 mg twice daily for 1-3 days. Chronic bacterial prostatis 300 mg twice daily for 6 wk. Typhoid fever 200-400 mg twice daily for 7-14 days.

Intravenous
Adult: 200 mg daily by IV infusion over at least 30 min. Max: 400 mg bid infused over at least 1 hr.

Renal impairment: Initial dose as normal then reduce.
CrCl (ml/min)
20-50 Reduce dose by half or give usual dose only every 24 hr.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
