Side-effects
- ভেরাপামিল সাধারণত ভাল সহ্য করা হয়। মৌখিকভাবে পরিচালিত ভেরাপামিলের নিম্নলিখিত প্রতিক্রিয়া স্পষ্টভাবে ড্রাগ সম্পর্কিত বা প্রায় 5000 রোগীর সাথে ক্লিনিকাল ট্রায়ালে 1% এর বেশি হারে ঘটেছে। পরিপাকতন্ত্র: কোষ্ঠকাঠিন্য

Drug Interactions
**ভেরাপামিলের সাথে ওষুধের মিথস্ক্রিয়া:**
- **বিটা-ব্লকার এবং ফ্লেকাইনাইড:** হার্টের ধীরগতির প্রভাব বাড়াতে পারে।
- **ডিগক্সিন:** ধীর হৃদস্পন্দন, হার্টের ছন্দের সমস্যা এবং ডিগক্সিন বিষাক্ততার উচ্চ সম্ভাবনা।
- **রেমিফেন্টানিল এবং সুফেন্টানিল:** ধীর হৃদস্পন্দন এবং নিম্ন রক্তচাপের ঝুঁকি বেশি।
- **Everolimus:** একসাথে ব্যবহার করলে উভয় ওষুধের মাত্রা বেড়ে যেতে পারে।
- **অন্যান্য ওষুধ (ডক্সোরুবিসিন, বাসপিরোন, কার্বামাজেপাইন, সাইক্লোস্পোরিন, এপিরুবিসিন, এপ্লেরেনন, কুইনিডিন, স্ট্যাটিনস, সিরোলিমাস, ট্যাক্রোলিমাস, কুইনুপ্রিস্টিন/ডালফোপ্রিস্টিন):** ভেরাপামিলের সাথে গ্রহণ করলে তাদের মাত্রা বেড়ে যেতে পারে।
- **অ্যালকোহল:** ভেরাপামিল রক্তে অ্যালকোহলের মাত্রা বাড়াতে পারে।
- **লিথিয়াম:** প্রভাবগুলি অপ্রত্যাশিত৷
- **ফেনোবারবিটাল, সালফিনপাইরাজোন, রিফাম্পিসিন, রিফাবুটিন, রিফাপেন্টাইন:** ভেরাপামিলের মাত্রা কমাতে পারে।
- **প্রোটেজ ইনহিবিটরস, সিমেটিডিন:** ভেরাপামিলের মাত্রা বাড়াতে পারে।
**গুরুতর বা জীবন-হুমকিপূর্ণ মিথস্ক্রিয়া:**
- **অ্যামিওডারোন:** হার্টের ধীরগতির প্রভাবকে ব্যাপকভাবে বৃদ্ধি করতে পারে।
- **ডোফেটিলাইড, রেনোলাজিন, সার্টিন্ডোল:** বিপজ্জনক হৃদযন্ত্রের ছন্দের উচ্চ ঝুঁকি (কিউটি প্রলম্বন)।
- **আইভাব্র্যাডিন:** এমনকি ধীরগতির হৃদস্পন্দনের কারণ হতে পারে।
- **ক্লোনিডাইন:** গুরুতর হার্ট ব্লকের ঝুঁকি বেশি।
- **ড্যানট্রোলিন:** প্রাণঘাতী উচ্চ পটাসিয়াম এবং হার্ট ফেইলিউরের ঝুঁকি।
Contraindications
নিম্নলিখিত ক্ষেত্রে ভেরাপামিল ব্যবহার করা উচিত নয়:
- বাম হার্ট পাম্পিং, খুব কম রক্তচাপ, বা কার্ডিওজেনিক শক নিয়ে গুরুতর সমস্যা
- সিক সাইনাস সিন্ড্রোম (যদি না রোগীর কৃত্রিম পেসমেকার থাকে)
- সেকেন্ড বা থার্ড-ডিগ্রি হার্ট ব্লক (যদি না কাজ করা কৃত্রিম পেসমেকার থাকে)
- অ্যাট্রিয়াল ফ্লাটার বা অ্যাট্রিয়াল ফাইব্রিলেশনের রোগী যাদের হার্টে অতিরিক্ত বৈদ্যুতিক পথ রয়েছে (যেমন উলফ-পারকিনসন-হোয়াইট বা লোন-গানং-লেভাইন সিন্ড্রোম)
- ভেরাপামিল হাইড্রোক্লোরাইডে অ্যালার্জিযুক্ত রোগী
যেসব রোগীরা বিটা-ব্লকার (ইনটেনসিভ কেয়ার ইউনিটে না থাকলে) গ্রহণ করছেন বা যাদের ভেরাপামিল হাইড্রোক্লোরাইডের প্রতি পরিচিত অ্যালার্জি আছে তাদের ভেরাপামিল ইনজেকশন শিরা (IV) দিয়ে দেওয়া উচিত নয়।
Mode of actions
ভেরাপামিল ধমনী, হৃদপিন্ডের পেশী এবং হার্টের বৈদ্যুতিক সিস্টেমের পেশী কোষে ক্যালসিয়ামকে প্রবেশ করতে বাধা দিয়ে কাজ করে। এটি করোনারি ধমনীতে খিঁচুনি থামাতে এবং প্রতিরোধ করতে সাহায্য করে, রক্তনালীগুলিকে প্রশস্ত করে রক্তচাপ কমায় এবং হার্টের উপর কাজের চাপ কমায়। দীর্ঘস্থায়ী অ্যাট্রিয়াল ফ্লাটার বা ফাইব্রিলেশন রোগীদের ক্ষেত্রে, ভেরাপামিল হৃদস্পন্দনকে ধীর করে দেয় এবং এটি দ্রুত হার্টের ছন্দের (প্যারোক্সিসমাল সুপ্রাভেন্ট্রিকুলার টাকাইকার্ডিয়া) আকস্মিক এপিসোড প্রতিরোধে সহায়তা করে। সামগ্রিকভাবে, ভেরাপামিল রক্তচাপ কমায়, বুকের ব্যথা (এনজাইনা) কমায় এবং হার্টের এভি নোডের মধ্য দিয়ে যাওয়া বৈদ্যুতিক সংকেতকে ধীর করে দেয়।
Dosage & Administration
মৌখিক: ভেরাপামিলের ডোজ টাইট্রেশন দ্বারা পৃথক করা উচিত এবং ওষুধটি খাবারের সাথে পরিচালনা করা উচিত।
অপরিহার্য উচ্চ রক্তচাপের জন্য প্রাথমিক ডোজ 180 মিলিগ্রাম সকালে দেওয়া উচিত। যদি 180 মিলিগ্রাম ভেরাপামিলের সাথে পর্যাপ্ত প্রতিক্রিয়া পাওয়া না যায় তবে ডোজটি নিম্নলিখিত পদ্ধতিতে টাইট্রেট করা যেতে পারে:
প্রতিদিন সকালে 240 মিলিগ্রাম।
প্রতি সকালে 180 মিলিগ্রাম এবং প্রতি সন্ধ্যায় 180 মিলিগ্রাম।
প্রতি 12 ঘন্টায় 240 মিলিগ্রাম।
এনজিনার জন্য সাধারণ ডোজ 80 মিলিগ্রাম থেকে 120 মিলিগ্রাম দিনে তিনবার।
ডিজিটালাইজড রোগীদের অ্যারিথমিয়াসের জন্য, অবস্থার তীব্রতার উপর নির্ভর করে ভেরাপামিলকে 240 মিলিগ্রাম থেকে 360 মিলিগ্রাম বিভক্ত মাত্রায় দেওয়া উচিত। 180 মিলিগ্রাম/দিন পর্যন্ত বিভক্ত ডোজ মাঝে মাঝে প্রয়োজন হতে পারে।
শিরায়: প্রাপ্তবয়স্ক: 5 মিলিগ্রাম ধীরে ধীরে শিরায়, টাকাইকার্ডিয়াস এবং উচ্চ রক্তচাপজনিত সংকটে, প্রয়োজনে 5 থেকে 10 মিনিটের পরে পুনরাবৃত্তি করুন।
থেরাপিউটিক প্রভাব বজায় রাখার জন্য ড্রিপ ইনফিউশন: 5-10 মিলিগ্রাম/ঘন্টা শারীরবৃত্তীয় স্যালাইন, গ্লুকোজ, লেভুলোজ বা অনুরূপ দ্রবণ, গড়ে 100mg/দিনের মোট ডোজ পর্যন্ত।
শিশু: নবজাতক 0.75-1mg (= 0.3-0.4ml)
শিশু 0.75-2mg (= 0.3-0.8ml)
1-5 বছর বয়সী শিশু: 2-3mg (= 0.8-1.2ml)
বয়স 6-14 বছর: ভেরাপামিলের 2.5-5mg (= 1-2ml), বয়স এবং কর্মের উপর নির্ভর করে শিরায় দেওয়া হয়।
ইলেক্ট্রোকার্ডিওগ্রাফিক নিয়ন্ত্রণের অধীনে ইনজেকশনটি ধীরে ধীরে করা উচিত এবং শুধুমাত্র প্রভাব শুরু হওয়া পর্যন্ত।
হাইপারটেনসিভ সংকটে শিরায় আধান: প্রাথমিকভাবে 0.05-0.1mg/kg/hour; যদি প্রভাব অপর্যাপ্ত বলে প্রমাণিত হয়, তবে ডোজটি 30-60 মিনিটের ব্যবধানে বৃদ্ধি করা হয় যতক্ষণ না ডোজ দ্বিগুণ বা তার বেশি পৌঁছায়। গড় মোট ডোজ 1.5mg/kg/day পর্যন্ত।
Pregnancy & Lactation
যদিও প্রাণীর প্রজনন অধ্যয়ন ভ্রূণের উপর বিরূপ প্রভাব দেখিয়েছে এবং গর্ভবতী মহিলাদের মধ্যে কোন পর্যাপ্ত এবং সুনিয়ন্ত্রিত অধ্যয়ন নেই, তবে গর্ভাবস্থায়, সম্ভাব্য ঝুঁকি থাকা সত্ত্বেও এই ওষুধটি ব্যবহার করা যেতে পারে যখন নিশ্চিতভাবে প্রয়োজন হয়।
একটি নবজাতক শিশুর মায়েরা যারা গর্ভাবস্থার শেষ 3 মাসে এই ওষুধটি ব্যবহার করেছেন তারা কদাচিৎ তন্দ্রা, পেশী শক্ত হওয়া বা ঝাঁকুনি, খাওয়ানো বা শ্বাসকষ্ট, ক্রমাগত কান্না সহ লক্ষণগুলি তৈরি করতে পারে। আপনি যদি আপনার শিশুর এই লক্ষণগুলির মধ্যে কোনটি লক্ষ্য করেন বিশেষ করে তাদের প্রথম মাসে, অবিলম্বে ডাক্তারকে বলুন।
