Tuberculosis, active or dormant, paradoxical bronchospasm, youngsters, the elderly, a pregnant woman, and a mother who feeds her infant breast milk.
Corticosteroids, including budesonide, are metabolized mostly through cytochrome P450 (CYP) isoenzyme 3A4 (CYP3A4). The mean plasma concentration of orally administered budesonide increased after oral administration of ketoconazole, a potent inhibitor of CYP3A4. Concurrent treatment of CYP3A4 may impede budesonide metabolism and increase systemic exposure. When using Budesonide Nasal Spray in combination with long-term ketoconazole or other known strong CYP3A4 inhibitors, caution is advised (such as ritonavir, atazanavir, clarithromycin, indinavir, itraconazole, nefazodone, nelfinavir, saquinavir, telithromycin).

Budesonide is a corticosteroid that primarily acts as a glucocorticoid. It works by slowing down protein synthesis, reducing the migration of polymorphonuclear leucocytes/fibroblasts, and reversing capillary permeability to prevent and regulate inflammation.
Adults and children 6 years of age and older: 100 mcg per day administered as one spray per nostril once daily.
The maximum recommended dose for adults (12 years of age and older) is 400 mcg per day administered as four sprays per nostril once daily.
Although animal reproduction studies have shown an adverse effect even sometimes unable to demonstrate the risk on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required.
