Precautions and warnings

Drug Interactions

Zuclopenthixol can make you feel more drowsy if you drink alcohol or take medicines that slow down your brain, like barbiturates or other sedatives. It can also make blood pressure medicines like guanethidine less effective. If you take zuclopenthixol together with metoclopramide or piperazine, you have a higher risk of movement problems (extrapyramidal symptoms). Using it with lithium or sibutramine increases the risk of serious damage to your nervous system. Taking other medicines that have anticholinergic effects with zuclopenthixol can cause stronger anticholinergic side effects (like dry mouth, blurred vision, constipation).

**Serious Risks:**
Zuclopenthixol can block the effects of medicines like apomorphine, levodopa, and other dopamine boosters, which can be dangerous. Taking zuclopenthixol with clozapine increases the risk of serious blood problems. There is also a higher chance of abnormal heart rhythms if you use zuclopenthixol with medicines that affect your heart’s rhythm (such as certain antiarrhythmics, erythromycin, or medicines that can lower your potassium levels, like thiazide diuretics).

Contraindications

Allergic reactions (hypersensitivity). People who are in a coma, such as those caused by alcohol, barbiturate, or opiate overdose; people with porphyria; and children.

Mode of actions

Zuclopenthixol strongly attaches to D1 and D2 receptors, as well as alpha-adrenoreceptors. It also has mild antihistamine effects and can block some serotonin activity.

Dosage & Administration

Adult: PO Psychoses Initial: 20-30 mg/day in divided doses. Maintenance: 20-50 mg/day. Max: 150 mg/day.

IM Acute psychosis As acetate ester: 150 mg, repeat if needed 2-3 days later. Max cumulative dose: 400 mg/course. Max number of inj: 4/course. Max duration: 2 wk.

Chronic psychosis As decanoate ester: Initial: 100 mg as test dose, followed after at least 1 wk by 200-500 mg or more, repeated at 1-4 wkly if needed. Max: 600 mg/wk. Inj >2 mL to be distributed between 2 inj sites.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
