Precautions and warnings

Using this medication for a long time can cause unwanted growth of germs that it cannot kill, including fungi. It can also raise eye pressure or cause glaucoma, which may damage the optic nerve, reduce vision quality, affect side vision, and lead to cataracts behind the lens. People who wear contact lenses should not use these eye drops while their lenses are in.

Drug Interactions

**Interactions and Side Effects:**

- This medicine can raise the levels of certain drugs in your blood, like clozapine, ropinirole, or theophylline.
- It can make blood thinners (like warfarin) and diabetes medicine (like glibenclamide) work more strongly.
- Methotrexate may become more toxic if taken together.
- The amount of this medicine in your blood may go up if you take probenecid.
- If you take this medicine with multivitamins or supplements containing iron, zinc, calcium, or with some antacids (with aluminum, calcium, or magnesium), it may not be absorbed as well.
- Using it with certain heart medicines (like quinidine, procainamide, amiodarone, sotalol), some antidepressants (TCAs), macrolide antibiotics, or antipsychotics can increase the risk of heart rhythm problems.
- Taking corticosteroids at the same time may raise the risk of serious tendon problems.
- Using NSAIDs can increase the risk of nervous system side effects.
- The levels of phenytoin in your blood may change.

**Serious Warning:** Taking this medicine with tizanidine can greatly increase tizanidine levels, leading to dangerously low blood pressure and strong sedative effects.

- Thiazide diuretics can increase blood sugar and lower potassium, especially when used with corticosteroids.
- There is a higher risk of stomach ulcers or bleeding if used with NSAIDs.
- The effect of blood thinners can change.
- You may need higher doses of diabetes or blood pressure medicines if you take them together.
- This medicine can lower the amount of salicylates and antimuscarinic drugs in your blood.
- Drinking alcohol can make stomach irritation worse.
- The effect of this medicine may be reduced when taken with carbamazepine, phenytoin, primidone, barbiturates, or rifampicin.
- Taking it with ciclosporin can increase the levels of both medicines in your blood.
- The effects of this medicine may be stronger in women who take estrogen or birth control pills.

Contraindications

Do not use this product if you are allergic to any of its ingredients. Avoid using it if you have herpes simplex or other viral infections, fungal infections (mycosis), glaucoma, or if you are a newborn. Also, do not use it for fungal diseases of the eyes or ears.

Mode of actions

Ciprofloxacin works by causing breaks in the double-stranded DNA of certain bacteria and stopping the action of DNA gyrase, an enzyme bacteria need to reproduce their DNA.

Hydrocortisone is a corticosteroid that reduces inflammation and weakens the immune response. It helps by stopping certain white blood cells (polymorphonuclear leukocytes) from moving to sites of inflammation and by reversing the increased leakiness of blood vessels. Hydrocortisone can also be used to replace the hormones that the body lacks in cases of adrenocortical insufficiency.

Dosage & Administration

Eye:
Corneal Ulcers: The recommended dosage regimen for the treatment of corneal ulcer is two drops into the affected eye every 15 minutes for the first six hours and then two drops into the affected eye every 30 minutes for the remainder of the first day. On the second day, instill two drops in the affected eye hourly. On the third through the fourteenth day, place two drops in the affected eye every four hours. Treatment may be continued after 14 days if corneal re-epithelialization has not occurred.
Bacterial Conjunctivitis:
The recommended dosage regimen for the treatment of bacterial conjunctivitis is one or two drops instilled into the conjunctival sac(s) every two hours while awake for two days and one or two drops every four hours while awake for the next five days.

Ear:
Four drops instilled into the affected ear twice daily for seven days. The suspension should be warmed by holding the bottle in the hand for one or two minutes to avoid dizziness, which may result from the instillation of a cold suspension. The patient should lie with the affected ear upward, and then the suspension should be instilled. This position should be maintained for 60 seconds. Repeat, if necessary, for the opposite ear.

Paediatric Use:
Ear: Safety and effectiveness of this suspension in pediatric (6 months of age and older) patients for ear application have been established.
Eye:
Safety and effectiveness of this suspension in pediatric patients for eye application have not been established.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Another study shows that this medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
