Precautions and warnings
Use with caution in people who have problems with their kidneys, liver, heart, or prostate, as well as those with paralytic ileus (blocked intestine), diabetes, Parkinson’s disease, or who are pregnant. Be careful if the patient has a history of blood disorders, reduced bone marrow function, seizures, dementia, or high cholesterol. For injections into muscle (IM), watch for low blood pressure, slow or irregular heartbeat, and slow breathing—monitor blood pressure closely. In teenagers, there is a higher risk of weight gain and high cholesterol, so use extra caution. The safety and effectiveness of this medicine have not been proven in children.
Side-effects
- Postural hypotension; constipation; dizziness; wt gain; agitation; insomnia; akathisia; tremor; personality disorders; oedema; somnolence; increased appetite; antimuscarinic effects; speech difficulty; exacerbation of Parkinson's disease; hallucinations; asthenia; increased body temperature; bradycardia; hyperprolactinaemia; QT prolongation (uncommon); asymptomatic elevations of hepatic transaminases. Potentially Fatal: Exacerbation of preexisting diabetes sometimes leading to ketoacidosis. Neuroleptic malignant syndrome.
Drug Interactions
Olanzapine can reduce the effects of levodopa and dopamine agonists. Medicines that increase certain liver enzymes, like omeprazole and rifampicin, can make the body remove olanzapine faster. Drugs that block these enzymes might slow down the removal of olanzapine. Taking carbamazepine can also make olanzapine leave the body more quickly. If you take activated charcoal at the same time as olanzapine, your body absorbs 50-60% less olanzapine. Be careful when using olanzapine with other drugs that affect the brain or with alcohol.
Contraindications
Angle-closure glaucoma; breastfeeding.
IM (intramuscular use): Use with caution if you have a history of heart disease or have had heart surgery.
Mode of actions
Olanzapine is a type of antipsychotic medication that works by attaching to several kinds of receptors in the brain. These include serotonin (5-HT2A and 5-HT2C), dopamine, muscarinic (M1 to M5), histamine (H1), and adrenergic (?1) receptors.
Dosage & Administration
Adult: PO Schizophrenia Initial: 5-10 mg/day adjust according to response. Usual: 5-20 mg/day. Max: 20 mg/day. Acute mixed or manic episodes in bipolar disorder Initial: 10 or 15 mg/day as monotherapy or 10 mg/day as part of combination therapy. Usual: 5-20 mg/day.
For prevention of recurrence: Start w/ 10 mg/day.
Pregnancy & Lactation
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
