Precautions and warnings

CHF, sick-sinus syndrome, severe ventricular dysfunction, hypertrophic cardiomyopathy, and severe aortic stenosis are all symptoms of impaired liver or renal function. In patients with idiopathic hypertrophic subaortic stenosis, use with caution. Children, the elderly. Lactation and pregnancy.

Drug Interactions

Amlodipine: When used with CYP3A4 inhibitors, plasma concentrations may be raised (for example, azole antifungals, ritonavir). Simvastatin and concomitant medication may raise the risk of myopathy, including rhabdomyolysis. Conivaptan and ciclosporin plasma levels may be increased.

Valsartan: When used with NSAIDs, it has the potential to counteract hypotensive effects and raise the risk of renal impairment. With K-sparing diuretics, K supplements, or K-containing salt replacements, there is a higher risk of hyperkalemia.

When administered together with aliskiren in individuals with diabetes and renal failure, there is an increased risk of hypotension, hyperkalemia, and abnormalities in renal impairment function (including acute renal failure).

Contraindications

Hypersensitivity to dihydropyridines is known.

Mode of actions

Amlodipine relaxes the smooth muscle of the peripheral and coronary arteries. During depolarization, it inhibits Ca ion entry into the slow channels or select voltage-sensitive channels of the vascular smooth muscle and heart, resulting in coronary vasodilation. In patients with vasospastic angina, it also improves myocardial oxygen delivery.

Valsartan is an angiotensin II type 1 (AT1) receptor antagonist that lowers blood pressure by blocking angiotensin II-induced vasoconstriction, aldosterone production, and sodium reabsorption in the kidney.

Dosage & Administration

Adult: PO Initiate with 5 mg/160 mg once daily.

May increase dose after at least 2 weeks, not to exceed 10 mg/day amlodipine and 320 mg/day valsartan once daily if needed.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Another study shows that this medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
